[Prolongation osteotomy of the humerus.].

The authors present results of treatment of asymmetric shortening of the humerus by prolongation osteotomy in a group of 6 patients (1 man and 5 women). Prolongation was indicated in three instances on account of shortening due to haematogenic osteomyelitis, in two instances on account of congenital hypoplasia and in one instance on account of posttraumatic shortening. The mean age of the patients at the time of operation was 14.2 years, the mean shortening was 9 cm, the mean correction 7.3 cm. The greatest prolongation was 10 cm, the smallest 6 cm. The period of treatment evaluated by the healing index was on average 27.3 days per 1 cm of regenerate. The follow up interval after operation was on average 16 months. In all patients prolongation osteotomy was performed semi-closed in the upper half of the humerus, in case of double osteotomy the supplementary osteotomy was performed in the middle of the humerus. In all patients complete correction of the shortening was achieved, As a rule the child apparatus Pro Spon of Uni-Fix Co. was used, in case of double osteotomy Ilizarov's apparatus. In one patient the authors recorded early postoperative neuropraxia of the radial nerve and complete recovery occurred 4.5 months after operation. No other serious complications were observed. The functional and cosmetic result was evaluated in all patients as excellent or very good. Key words: prolongation of the humerus, semi-closed osteotomy.
