[Contribution to the Solution of Patello - femoral Complaints in Children and Adolescents.].

The authors give an account of their experience with surgery of relapsing dislocations of the patella in children and adolescents. They evaluate a group of 82 patients (89 knee joints) who were attended and treated at the Orthopaedic Clinic of the Faculty Hospital in Hradec Králové between January 1990 and August 1997. In all patients arthroscopy was made which rendered the diagnosis more accurate and made it possible to select the optimal therapeutic procedure. If necessary, revealed intraarticular damage was treated arthroscopically. In 42 patients simple release of the lateral retinaculum of the patella was made. In 49 cases combined plastic surgery of the suspensory apparatus of the patella was performed involving a modified combination of Roux-Goldthwaite and Campbell's procedures. The described combination proved very satisfactory. It corrects the major portion of non-physiological components and makes early postoperative rehabilitation possible. The authors did not observe in their group any re-dislocations or other serious complications. Key words: dislocation of the patella, arthroscopy in childhood patellar instability.
