[Intraarticular fractures of the distal aspect of the radius.].

Intraarticular fractures of distal radius are caused often by high energy trauma in patients of middle and younger age. These fractures have to be distinguished from extraarticular fractures and treat them accordingly. Even a small incongruency of the articular surface of radius may lead to posttraumatic osteoarthritis. The choice of the method of treatment is determined mainly by the type of fracture, its stability and degree of dislocation. Surgical methods include open reduction and internal fixation, external fixation, application of bone grafts. Semiconservative can be considered the method of percutaneous fixation by K-wires. Recently an increasing attention has been devoted to the so called associated injuries in the region of distal radioulnar joint. The latest trend in the treatment is the combination of techniques in order to achieve an accurate reconstruction of the joint surface. The article present evaluation of a group of 19 patients hospitalized at our clinic with the diagnosis of intraarticular fracture of distal radius and treated by the semiconservative or surgical method. The average age of patients was 44 years, of which 12 were men. Ten times we used external fixator, seven times K-wires, twice a combination of methods. The result was evaluated as excelent - 9times, good - 5times, fair - 3times, poor - twice. The author presents also complications of these severe fractures.
