[Percutaneous nucleotomy. 10 years' experience, modern concepts].
H Leu, A Schreiber
Acta Chir Orthop Traumatol Cech. 1990; 57(2):97-110
The article reviews the authors' ten-year experience with the technique of percutaneous nucleotomy in the treatment of lumbar disc herniation. During this period 174 operations were altogether performed in 148 patients by means of this technique. The existing experience shows that this technique has fully justified itself in the indicated cased. It can be applied for the protrusion and subligamentous extrusion of the disc. Subligamentous sequestrum represents relative indication. For the evaluation of the condition of the disc the authors use as a standard the technique of contrast discomanometry. This examination method they consider in the indication...
[Extensive subtalar release in the treatment of congenital pes equinovarus].
P Dungl, M Karpísek
Acta Chir Orthop Traumatol Cech. 1990; 57(2):111-126
The results of the surgical treatment by the method of extensive subtalar release after McKay is observed on the group of 35 feet operated on in 23 patients. In 17 feet it was the case of the recurrence after the operation, 18 feet were operated on primarily either after the conservative therapy or after no preceding therapy. Surgical technique is described in which major stress is put on the considerate approach and perfect correction of all abnormalities within one operation. A three-grade evaluation was used for the assessment of the results: good, fair, poor. Good results were achieved in 60 per cent of operated feet, 23 percent were evaluated...
[Compartment syndrome in fractures of the shin].
J Kolman, M Slavík, B Soukup
Acta Chir Orthop Traumatol Cech. 1990; 57(2):127-137
Compartment syndrome represents the most serious complication in the treatment of the fractures of the shin. In the period 1979-1987 17 patients with variously developed form of the compartment syndrome were treated at the Orthopaedic clinic of the Institute for In-service Education of Physicians. 7 patients, i. e. 40 per cent were transported from other clinics. There were 8 open fractures in the group, i. e. 47 per cent, and they were classified according to Allgöwer. All patients were primarily operated on for the fracture of the shin and the compartment syndrome occurred in 11 cases after plate osteosynthesis and in 6 cases as a result of the implementation...
[Care of chronic rupture of the fibular ligaments using the Weber method of reconstruction at the ILF].
P Vavroń, M Slavík
Acta Chir Orthop Traumatol Cech. 1990; 57(2):138-143
After an inadequate conservative treatment of III-grade sprain of the ankle combined with the rupture of ligaments, these heal in prolongation which result in the chronic instability of the ankle joint. After the evaluation of the anamnesis and the objective examination the diagnosis is specified by comparative forced X-ray images in both projections. To substitute insufficient fibular ligaments we use a modification of the Weber method. We perform tenodesis of m. peronaeus brevis. The tendon is led through the bone channel above the apex of the lateral maleolus and its proximal end is fixed in the area of the origin of lig. fibulotalare anterius to...
[Sternoclavicular dislocation. Case report].
I Kopecký, E Holec
Acta Chir Orthop Traumatol Cech. 1990; 57(2):144-150
The authors describe two cases of the disease. They perform modified operation after Speed in which they use instead of a strip from fascia lata a 1 mm thick binding wire for the fixation of the medial end of the clavicle to the first rib. Immobilization in the Desault bandage takes three weeks and is followed by rehabilitation with the reduction of elevation above 90 degrees. After twelve weeks the upper extremity of the affected side can perform normal activity, however, with permanent exclusion of weightbearing of the extremity in the direction conductive to dislocation.
[Use of cement plugs in the care of giant cell bone tumors].
I Kofránek, Z Matĕjovský, V Frühaufová, Z Hedvábný, V Trnka
Acta Chir Orthop Traumatol Cech. 1990; 57(2):151-160
The classical giant cell tumor represents a problem as the clinical, radiological and histological parameters not always exclude the tendency to local recurrences, rarely also to metastases. Surgical treatment of this tumor is characterized by a relatively high percentage of local recurrences. In the years 1965-1987 patients with 72 giant cell tumors were treated within the Prague Team for Bone Tumors. In 1980 the existing results were revised in 37 patients operated on to that time and the therapeutical scheme was changed by the introduction of new methods mainly the application of bone cement. While in 17 patients we have primarily operated on in...
[Comparison of treatment of juvenile bone cysts using local administration of corticoids and surgical therapy].
V Stědrý, Z Matějovský
Acta Chir Orthop Traumatol Cech. 1990; 57(2):161-167
The authors describe the results of the juvenile bone cyst in the group of 41 patients by the method of the application of corticoids and compare them with the results of the therapy by means of excochleation and filling the cavity of the cyst with bone grafts. In the therapy by corticoids restitutio ad integrum was achieved in 37.8 per cent and in 43.2 per cent of patients the healing was incomplete. In 5.5 per cent of cases there occurred recurrence after the cyst was healed, in 13.5 per cent there was no response to the application of corticoids. In the groups of surgically treated patients the cyst was healed in 68.8 per cent, out of which 48 per...
[Early diagnosis of congenital hip dislocation in neonates].
J Skrabal
Acta Chir Orthop Traumatol Cech. 1990; 57(2):168-177
On the basis of the analysis of the examination of hip joints in 1000 newborns the author supports his former statement that of almost the same frequency as the acknowledged symptomatology of unstable hip joint (in 6.5 per cent) is the pathological symptomatic group of increased tension of tendons of the origin of adductors (in 4.7 per cent), sometimes also with reduced abduction, less palpable head of the hip joint and the symptoms presented by him can improve the quality of screen position case of breech presentation. This was found in 5.2 per cent of deliveries. The combination of both symptomatic groups occurs in 1.4 per cent. Out of individual...
[Conservative containment in the treatment of Legg-Calvé-Perthes disease].
M Rejholec, J Vosátka, K Sadil
Acta Chir Orthop Traumatol Cech. 1990; 57(2):178-184
The article presents results of the conservative containment treatment by means of abduction splint the Scottish Rite Hospital--Atlanta: achieved in the group of 21 children three to eight years old. The mean duration of the observation is 27 month (12 to 48 month). In 13 cases (62 per cent) the treatment was done merely by the application of the abduction splint, in 9 cases the application of the splint represented a preparatory phase for the surgical treatment according to the principles of containment. For the main objective of the treatment the authors consider the necessity of the preservation of the spherical shape of the head or remodellation...
