[Principles of thermography and its application in medicine, and mainly in orthopaedics.].

T Trc

Acta Chir Orthop Traumatol Cech. 1992; 59(1):3-7

The autor deals with termography as a modern diagnostic tool from the viewpoint of physical regularities, the major historical events relating to infrared radiation and the ways of its detection. On the basic of physical regularities and historical knowledge he presents the application of thermography also in medicine. He describes the basic thermodynamic regularities and processes taking place in the human body and the possibilities of their detection by infrared camera. In details he deals with orthopaedic diseases the reaction of which on the termic regimen of the body can be monitored by termography. The article is an introduction to concrete studies...

[Contribution of thermography to the diagnostics of entesopathy of epicondyli lateralis humeri.].

T Trc

Acta Chir Orthop Traumatol Cech. 1992; 59(1):8-10

The author describes results of thermographical examination of patients with the affected lateral epicondyle of humerus prior to the treatment and after the treatment, both conservative and surgical. He descibes both the normal and pathological thermogram of the elbow and evaluates the negative findings in cases of clinically diagnosticated entesopathy of lateral epicondyle and the subsequent treatment. In case of negative thermographic findings on epicondyles there often occur pathological findings in the field of cervical spine indicating a cervicobrachial syndrome with a subsequent secondary entesopathy. Key words: thermography, entesopathy, epicondylitis,...

[Arthrogryposis multiplex congenita. I.].

I Marík, A Schejbalová, M Kuklík, J Kraus

Acta Chir Orthop Traumatol Cech. 1992; 59(1):11-18

In part I the authors deal with the present possibilities of the diagnostics and therapy of arthrogryposis multiplex congenita. They proceed from the classification after Hall of 1982 (classification of the so-called distal arthrogryposis) and of 1983 where congenital contractures are divided into three groups according to associated system anomalies. On the basis of their own experience they deal in details with distal arthrogryposis - amyoplasia which differs from spinal degenerative diseases by 3 symptoms: 1. it does not deteriorate after the birth, 2. nerves are not affected, 3. intelligence is normal. Differential diagnostics is carried out on...

[Treatment of closed fractures of the shin external fixation.].

J Stehlík, O Cech, M Dlouhý

Acta Chir Orthop Traumatol Cech. 1992; 59(1):19-22

In their communication the authors present a method used for the treatment of closed fractures of the shin by the method of external osteosynthesis, its indication criteria and the actual technique of operation. Open reduction is the method of choice in cases of early treatment of instable fractures of the shin with a good condition of skin cover and other soft tissues. Closed reduction is the method of choice in those patients the treatment of which is postponed (by more than 3 weeks) and in fresh injuries combined with a more extensive damage of soft tissues associated with oedema of the extremity often with a segmental type of fracture. For a very...

[Reoperation of Cemented Total Endoprosthesis of the Hip Joint Part I. X-ray Diagnostics and the Classification of Loosening.].

M Krbec, O Cech, V Dzupa

Acta Chir Orthop Traumatol Cech. 1992; 59(1):23-26

The authors monitored a group of 87 patients after a reoperation of the total replacement of the hip joint due to the so-called aseptic loosening of one of the components. On the basis of the evaluation of clinical complaints and an x-ray finding of the extent and progress of the so-called radioluscent line on the cement-bone interface and their correlation with the peroperative finding of the loosening of the implant they developed an x-ray classification of the loosening of the total endoprosthesis. This three-grade classification facilitates the consideration of the latest state of the fixation of the implant and the prognosis of further development....

[Use of ultrasound in the early diagnosis of the syndrome of congenital dysplasia of the hip joint.].

M Matejícek

Acta Chir Orthop Traumatol Cech. 1992; 59(1):27-30

The early diagnosis and correct treatment of the syndrome "Dysplasia coxae congenita" (DCC) is one of the fundamental chapters of orthopaedic child care. The author evaluates a group of 150 children examined by sonography to assess the possible presence of the DCC syndrome during the first trimester of life. The sonographic examination was made in three time intervals defined in advance: 1st check-up during the first-second week after birth; 2nd check-up at the age of 6-7 weeks and 3rd check-up at the age of 13 - 14 weeks. During the final evaluation a relatively large number of pathological findings was found at the age of 1-2 weeks, roughly in one...

[Non-specific Spondylodiscitis in Children.].

J Blaha, K Karpas

Acta Chir Orthop Traumatol Cech. 1992; 59(1):31-36

The author deals with non-specific spondylodiscitis in children which occurs relatively rarely. A group of 6 children (age range 6-15 years) has been gathered from the material of the Orthopaedic Clinic in Hradec Králové in which diagnostics and treatment are dealt with in details. Diagnosis is based on anamnestic, laboratory, clinical data, x-ray examination and results of scintigraphy. Forage biopsy is used and the material fom the forage is histologically examined. Diagnostics and treatment is presented in the chart. In the discussion the authors confront their own results with literary data. The connection between the origin of the disease and...

[Results of sonographic Examination of Some Tumours and Tumorous Affections of Soft Tissues of the Musculoskeletal Apparatus.].

J Pelnár, D Hrusák, T Pribyl

Acta Chir Orthop Traumatol Cech. 1992; 59(1):37-40

On the basis of a selected group of eight patients the authors compare sonographic, clinical, peroperative and histological findings of tumours and tumorous affections of benign nature of soft tissues of the musculoskeletal apparatus. For a major contribution the authors consider the fact that it is a non-invasive graphic method convenient for the patient by which location and size of the tumour can be determined, as well as the relation of the mass to surrounding structures and to a certain extent also the consistency of the mass on the basis of the so-called A mode. However, the significance of sonography should not be overstimated. It is a com plementary...

[Our experience in the surgical treatment of pseudoarthrosis of scaphoids in the period 1977 - 1987.].

J Husták, K Foldyna

Acta Chir Orthop Traumatol Cech. 1992; 59(1):41-44

The authors present an analysis of the results of the surgical treatment of pseudoarthorsis of scaphoid by the technique after Matti-Russe consisting in spongioplasty and bridging the pseudoarthrosis with corticospongy graft in 35 patients treated in the period 1977-1987. Bone healing was achieved in 83 per cent, in 63 per cent there was a significant remission of subjective complaints. In 57 per cent the osteoarthritic changes of carpus made progress, out of which in 50 per cent there were preoperatively present symptoms of carpal instability, i. e. scapho-lunate dissociation of more than 2 mm and/or increase in volar flexion of scaphoid or its fragments...

[Antibodies to Collagen in the Patients with primary Operation and Reoperation of Endoprosthesis of the Hip Joint.].

R Becvár, J Novotná, M Krbec, O Cech, M Adam

Acta Chir Orthop Traumatol Cech. 1992; 59(1):45-47

Using ELISA method titres were determined of antibodies to collagen of type I, II and III in the serum of the patients who underwent implantation or reoperation of total endoprosthesis of the hip joint. The group of primary operations included mainly patients with osteoarthritis (OA), ankylosing spondyllitistis (AS), rheumatoid arthritis (RA), the group of the reoperated comprised also patients after various traumas of the hip joint. Increased titres were generally found in all patients, however, in the group of reoperated patients the average values were higher than in the patients after primary operations. In the reoperated patients the level of...

[Clinical manifestation of spinal tumours after a trauma.].

S Grammetbauerová

Acta Chir Orthop Traumatol Cech. 1992; 59(1):48-52

Medullary tumours manifest themselves clinically after a certain asymptomatic period. This period is characterized by the oedema of spinal cord and the disorder of microcirculation at the level of compression. After a trauma of the back there occur biochemical processes which result among others in the accumulation of calcium ions and in the activation of metabolism of arachidonic acid and thus in an increased synthesis of prostanoids. These agents lead up to the deterioration of vasogenous oedema at the level of compression and the tumour clinically manifests itself in connection with the trauma. The author analyzed a group of 66 patients with a medullary...

[Report on the influence of some materials on the proliferation of cells.].

V Pesáková, S Beznoska, O Cech, M Adam, M Krbec

Acta Chir Orthop Traumatol Cech. 1992; 59(1):53-57

The authors have examined the biocompatibility of four materials used for the production of implants with the proliferation of cells. This applies mainly to Ti-6 AI-4 V, Ti- 5AI-2,5 Fe, AKV-Ultra 2 alloys and also to the carbon filament connected by polyamide. In their study the authors used diploid clone -human embryonal fibroblasts LEP and heteroploid lines - Vero cells. It has been found out that neither of the materials tested is of a toxic nature, however, under certain conditions there occurs the reduction of proliferation abilities of cells most probably due to the influence of agents which have passed to the cultivation medium. Key words: fibroblasts,...
