[Initial Experience with a New Generation of Zweymüller's Endoprosthesis.].

In the first part of our paper innovations as regards the non-cemented endoprosthesis of the hip joint by Zweymüller's method are described. As a special advantage the double cone of the socket is mentioned which as to its anatomical shape is close to the acetabulum and makes only slight resection of the bone on the floor of the acetabulum possible. Ever since percutaneous lumbar nucleotomy and discoscopy were introduced great hopes in this method were raised by specialists and patients. Based on the initial euphoric comments in the professional press, the authors started to use this procedure in 1992. During the period between Jan. 1 1992 and Dec. 31 1993 at the orthopaedic department of the General Hospital in Zwettel 69 operations of the disc were performed. Reducing the Stress in the Articular Surface of the Hip Joint after Shifting the Upper Part of the Body towards the Painful Hip.

In this work the change of stress distribution in the hip joint articular surface after shifting of the upper part of the body towards the painful hip due to pain reflex is estimated theoretically by using a three dimensional mathematical model. It is shown that after displacement of the upper part of the body towards the painful hip the stress in the hip joint can be in case of poor femoral head coverage and small inclination of the pelvis very little decreased in spite of the fact that the resultant hip joint force is reduced considerably. This part of the paper is based on the theoretical background and principles of conservative and surgical treatment described in part I. The authors describe the principle of operation and give a detailed account of it, supplemented by diagrams. In the discussion the authors mention views regarding the type and scope of surgery with regard to the assumed increased pressure exerted on the head of the femur, and also changes of the ratio of forces in the femur are discussed. [Value of peroperative biopsy in the diagnosis and treatment of musculoskeletal tumors.].

In the period 1982-1992 the authors have performed 1276 peroperative biopsies from frozen sections in 607 patients with suspect diagnosis of a musku-loskeletal tumor. In 174 a diagnosis of a primary malignant tumor was established, in 166 of a benigne tumor, in 53 of metastasis, in 104 of a tumor-like lesion, in 33 of an inflamation. In 77 patients (12,6 %) it was not possible to establish peroperatively the diagnosis and it became necessary to wait for the definite histological examination. [Epidemiology of damage of the meniscus in miners.].

The authors evaluated the health records of men treated by open arhtrotomy because of damage of the menisci of the knee joint in the Municipal Hospital in Ostrava in 1988-1989. The group was formed by miners working underground and a second group of patients who were not miners. The stratification of the two groups was made by age, circumstances under which the first symptoms developed, the surgical macroscopic and microscopic picture. The investigated group comprised 207 patients treated at the spondylologic consulting centre of the orthopaedic clinic where 576 assessment were made with subsequent computer evaluation. Mathematically the correlation between the angle of the costal hunch assessed by a scoliosometer and Cobb's angle on the X-ray picture was calculated. The value of the correlation coefficient was 0.8049. The authors demonstrate a relatively rare case of spontaneous healing of the navicular bone almost four years after the injury.
