[Growth of the components of the human hip joint.].

M Doskocil

Acta Chir Orthop Traumatol Cech. 1995; 62(3):133-141

The mutual ratio of the extent of cartilaginous surface of the acetabulum and of the femoral head in human fetuses from the second and third month, from fetuses from the 24th week of pregnancy and from human newborns, children and adults was studied. The results of our research are as follows: 1. In small human fetuses is the hip joint an ellipsoid articulation, the head lies deeply in the articular pit, the labrum glenoidale is large. 2. In fetuses from the 3rd quarter of pregnancy is the femoral head relatively big and it is situated only shallowly in the articular cavity, so that an impor tant part of the femoral head has no contact with the corresponding...

[External fixation in complicated fractures of the femur.].

J Stehlík, J Bartonícek, J Váchal

Acta Chir Orthop Traumatol Cech. 1995; 62(3):142-154

This work demonstrates the results of treatment of 14 patients with complicated injury of the femur. In all cases external fixation, type Poldi 7 was used. Of great importance from the viewpoint of treatment is the following: - proper revision with sequestrectomy and resec tion of necrotic fragments; - exact technics of aplication the external fixation; - adequate stabilisation of the modification of de vices; - necessity the changes between the type modifica tions Poldi 7 devices; - early and usually repeated cancellous bone graf ting. The evaluation shows the difficulty of treatment and also longterm healthy, social and famillies consequences. Key...

[Initial experience with arthroscopy of the joint of the hand.].

M Weissinger, C Helmreich

Acta Chir Orthop Traumatol Cech. 1995; 62(3):155-161

In the submitted paper the authors give an account on the development of arthroscopy of the joint of the hand and comments on arthroscopic anatomy and technique of the procedures. In the authors' hospital the procedure is carried out under general anaesthesia, the patient lies on his back with the arm suspended in extension by the fingers, using a 3-5 kg weight. The authors use a needle arthroscope of Storz Co. with a diameter of 2.7 mm which is inserted into the joint from a dorsomedial approach. During the period between Jan. 1, 1990 and Dec. 31, 1993 14 arthroscopies of the joint of the hand were made in 10 women and 4 men, three times on the left...

Medial Flatness of the Femoral Trochlea in the Nail Patella Syndrome.

J Vanek, L L Fournier

Acta Chir Orthop Traumatol Cech. 1995; 62(3):162-164

The knee joint of an adult male with a nail patella syndrome was examined by standard X-rays and by computerised tomography. The dysplasia of the femoral trochlea has a specific sign which distinguish it from the "common" dysplasia of the recurrent patellar luxation type: a straight line flatness of the medial margin on the profile. This sign should be useful for the diagnosis of incomplete forms of the nail patella syndrome. Key words: nail patella syndrome, osteo-onycho-- dysplasia, knee dysplasia.

[Five years experience with triple osteotomy of the pelvis, as described by steel.].

K Foldyna, J Musialek, R Boglevský

Acta Chir Orthop Traumatol Cech. 1995; 62(3):165-177

The authors present their five-year experience with triple osteotomy of the pelvis, as described by Steele, in a group of 31 patients and 35 operated hip joints. The mean age of the patients was 20 years, the mean follow up period 28 months (4 - 55). Prerequisites of surgery according to the authors are: 1. congruence of the head of the femur and aceta bulum, spherical or aspherical, before operation; 2. the possibility to achieve spherical or aspherical congruence by surgery; 3. sufficient functional range of movements in the hip joint, in particular inner rotation. The subjective satisfaction of patients with surgery was 79%, objective evaluation...

[Thoracoabdominal operations in patients with multiple injuries.].

J Zelenák, M Hutan, K Kálig

Acta Chir Orthop Traumatol Cech. 1995; 62(3):178-181

The authors analyze during a two-year period a group of 58 patients with multiple injuries, including a thoracoabdominal injury, with a total mortality of 29 %. The dominating injury was damage of the chest and the most frequent surgical procedure negative pressure drainage of the pleural cavity, as described by Büllau. 82% of the surgical procedures were performed during the acute stage of treatment of the multiply injured patient (unstable patient) within 8 hours after admission to hospital. If peroperative coagulopathy develops, the authors recommend intraabdominal packing, closure of the abdominal cavity and after the patient's stabilization (haemodynamics,...

[Rearthroscopy of the knee joint.].

Z Kostál, J Musialek, J Husták, M Macák

Acta Chir Orthop Traumatol Cech. 1995; 62(3):182-185

The authors evaluate a group of 1409 arthroscopic operations and 69 reoperations of the knee joint in the course of six years'activities. They analyze the causes which led to the reoperation and the most frequent findings recorded during rearthroscopies. They present their views on indication and timing of reoperations based on experience they have assembled. Key words: arthroscopy, rearthroscopy.

[Partial arrest of growth after intra partum physeal injury.].

P Havránek, V Mrzena

Acta Chir Orthop Traumatol Cech. 1995; 62(3):186-191

Treatment of the sequelae of the osseous connection between the metaphysis after damage of a part of the growth plate (bone bridge, partial arrest of growth) is still difficult. In addition to many symptomatic operations (repeated osteotomies, prolongation of the bones, epiphyseal distraction, epiphyseodesis etc.) also causal provisions can be made. Because transplantations of the physis are not yet very successful in clinical work, the only procedure which has an impact on the cause of restricted growth is resection of the bone bridge and interposition of inert material prevents a relapse of ossification at the site of the impaired physis. The position...
