[Experience with dynamic hip screw in retrochanteric and subtrochanteric fractures of upper femur.].

From 1st January 1988 to 31st December 1994, 105 patients were given 106 dynamic hip-screws in the Orthopaedic Department of the Public General Hospital, Zwettl. The average age at the time of operation was 77.6 years and the time lapse between the accident occuring and the operation performed was on average 31 hours. In 94.3% of the cases we could perform a closed reduction and in 88.7% we used a 135 degree-DHS. The four-hole plate dominated in 73.6% of the plates used. Internal complications totalled 23.6% out of which 9.5% of the patients died during their hospitalization. Here one must take note that the patients who died were of advanced age and...

The authors focus attention on the quality of osseous tissue in patients with revisions of TEP of the hip joint. They evaluate the quality of the osseous tissue indirectly, based on densitometric examination DEXA (DPX) using an apparatus LUNAR DPX L. They used software for whole body assessment. Measurements in 53 patients (33 women and 20 men), mean age of women 65 years, of the men 68 years, with an average period of 11 years (1-15 years) since the primary implantation revealed the following results. All patients have a reduced bone density of the pelvis and lower extremities. In men the density is lowest in the lower extremities - BMD 1.27 g/cm2...

[Arthroscopic surgical technique in relapsing dislocations of the shoulder joint.].

The author describes in the submitted paper his own arthroscopic surgical technique used in relapsing and habitual dislocations of the shoulder joint. This technique is based on producing a special surgical tool (modified Kirschner wire) and refixation of the lesion of the labrum with the suture and the raphe of the bursa of the shoulder joint. The author used this method in 27 patients with the above diagnosis. After two-year follow-up he recorded a single relapse. The asset of the technique is that it is simple, not demanding on instruments required, that it can be implemented in the out-patient department without hospital admission, convalescence...

[Late results of conservative treatment of perthes disease.].

The objective of the submitted work was to assess the long-term prognosis of Perthes disease. From the originally invited 142 patients 72 attended the check-up examination, complete X-ray documentation was assembled only in 32 patients with 38 affected hip joints. The time interval which had elapsed since the onset of the disease was on average 17 years. The authors evaluated the clinical and X-ray picture of the hip joints and assessed retrospectively the affection of the head according to Catterall's method, incl. signs of a head associated with risk. [Late results of early operations of congenital defects of the hands.].

The submitted paper is based on a group of 610 patients suffering from congenital defects of the hands. This number comprises 322 cases of syndactyly (s. simplex - 263, s. complicata - 59, s. ossealis - 9, acrosyndactylia - 6, s. totalis - 10, Sy Apert - 3, synbrachydactylia - 34, Sy Poland - 15), 23 cases of synpolydactylia, 265 cases of polydactylia. The authors describe their own 25-year experience with surgical treatment of different defects and evaluate the long-term postoperetive results of early operations of these patients. In the conclusion they maintain that surgical treatment of inborn defects of the hands must be evaluated individually...

In the introduction the authors give an account of immunization against tuberculosis in post-war Czechoslovakia up to the present time from the aspect of the epidemiologist. After a 14-year interval the authors evaluate the results of the investigation and treatment of children with osseous complication after immunization with the BCG vaccine type Moscow. The group comprises four children who were hospitalized years ago at the Orthopaedic Clinic in Hradec Králové for assessment of the diagnosis and initiation of comprehensive treatment. [Indications and Application of a Standard and Long Gamma-nail.].

An alternative treatment of fractures of the coxal femur side is a new implant - gamma-nail. Furthermore, we present the results of study of 77 patients, mainly women (71,4%) with an average on age of 65,5 years. In 52 percent, we found relevant instabilities (AO groups 31 - A2.2, A2.3 and A3.3). The modalities of treatment and mobilization are described, and the clinical and radiological results are presented. Mobilization was possible in 84,2% of these cases. The 6,5% of operative and postoperative complications we have to face, are described in detail. Key words: bone nails, femoral fractures, hip fractures.

The authors present a review of contemporary views on the problem of alloplasty of the shoulder joint. In the historical introduction they mention the development of surgery of the shoulder joint with the gradual transition from resection or interposition arthroplasties to endoprostheses of varying design which according to the principle of internal constraint can be divided into three groups, i. e. not compressed, semi-constrained and fully-constrained implants.
