[Tricalcium Phosphate as a Bone Tissue Substitute (testing of biological properties in animal experiments.].
K Urban, Z Strnad, C Povýsil, P Sponer
Acta Chir Orthop Traumatol Cech. 1996; 63(1):16-20
The authors present the results of implantations of tricalcium phosphate of LASAK Co. Prague into defects of the long bones of 12 experimental dogs after an exposure time of 2,4 and 8 months. Seven modifications of the mentioned material were used in the form of granules, prisms and powder in a total of 18 implantations. In all instances specimens after implantation were taken which did not have macroscopic or microscopic signs of inflammatory reaction. X-ray did not reveal any clear spots round the material, signalizing an inflammatory of fibrous reaction. Microscopic examination confirmed excellent tolerance of the surrounding live tissue and osteoblastic...
[Osteoid osteoma on the floor of the acetabulum: pitfalls of diagnosis and therapy.].
J Kiliján, P Dungl, H Zídková
Acta Chir Orthop Traumatol Cech. 1996; 63(1):21-23
The authors present the case of a rare localization of an osteoid osteoma on the floor of the acetabulum and describe the pitfalls of diagnosis and treatment of this benign tumour. On the case-history of an 11-year-old patient they demonstrate that it is correct to use radical surgery of this tumour in a rare localization. In the world literature so far only eight cases of osteoid osteoma in the area of the acetabulum are described. All authors agree that radical surgery is indicated. Key words: osteoid osteoma.
The "jump sign" of the cadaver knees after the transection of the superficial medial collateral ligament.
J Vanek
Acta Chir Orthop Traumatol Cech. 1996; 63(1):24-26
We present a new test for the examination of the external rotatory instability of the knee caused by the section of the superficial medial collateral ligament. The test consists in a manoeuvre by which the proximal end of the tibia is pushed laterally and the medial femorotibial compartment is exposed to varus rotation stress. In the majority of cases, this causes an abrupt anterior slip of the medial tibial plateau accompanied by a click. The test was verified on 33 cadaver knees. It may be helpful in the differencial diagnosis of knee instability and intraarticular disorders. Key words: knee instability, medial collateral ligament.
[Needle aspiration cytology of soft tissue tumours of the locomotor apparatus.].
J Dlask, A Ryska, Z Kerekes
Acta Chir Orthop Traumatol Cech. 1996; 63(1):27-30
The authors present an account of their five-year experience and results as regards the diagnosis of soft tissue tumours of the locomotor system suspect of malignity, using the method of needle aspiration biopsy. It is a semi-invasive examination with cytological evaluation of the aspirate from the tumour foci. The representativeness of the collected sample is, depending on anatomical conditions, checked by sonography. Thus to a minimal extent the physiological borders of the affected anatomical compartment are interfered with. From the total number of 65 punctures made in 1989-1994 the authors recorded a 94.4% sensitivity and 97% specificity of results.The...
[Iatrogenic gonitis.].
L Jencíková, G Vasko
Acta Chir Orthop Traumatol Cech. 1996; 63(1):31-33
The authors draw attention to the problem of iatrogenic gonitis. They analyze the causes of their development and draw attention to the possible negative effect of intraarticular therapy in patients with affections of the joints in orthopaedics. During the past seven years they recorded 54 cases of purulent iatrogenic gonitis after administration of corticoids and chondroprotective substances administered in out-patient departments. They emphasize the very rapid therapeutic effect of corticoids, while also side-effects of these preparations must be taken into consideration, in particular the development of pyogenic arthritis. Only satisfactory knowledge...
[Iruxol ointment - possibilities of healing soft tissue defects.].
D Zítko
Acta Chir Orthop Traumatol Cech. 1996; 63(1):34-36
Healing of skin defects can be a problem in contusions, open fractures and always in chronic lesions, in particular when combined with vascular and metabolic disorders. In addition to mechanical removal of necroses it is possible to use at present also the specific action of collagenases (1). The ointment Iruxol mono of Knoll Co. was used for the treatment of four patients with soft tissue defects of varying origin. The results provide evidence that local treatment with collagenases hastens healing and can save the patient from a plastic operation. Key words: soft tissue defects, collagenases.
[Pathological fracture of a lumbar vertebra in psoriatic spondylarthropathy.].
M Lutonský, K Karpas, A Chaloupka
Acta Chir Orthop Traumatol Cech. 1996; 63(1):37-39
The paper deals with a rare case of fracture of the lumbar vertebra in psoriatic arthropathy which finally led in a 39-year old female patient with a 14-year-old history of the disease to straightening of the spine. The fracture of the vertebral body L2 with straightening of the lumbar section by 25 degrees healed after conservative treatment without any complications. The case is discussed from the aspect of the cause of the fracture, mechanism of the episode. The resulting position of the spine is compared with the results of surgery, i. e. rachotomy. Key words: psoriatic spondylarthropathy, pathological fracture of the lumbar vertebra, rachotomy.
[Osseous bridges (case report).].
S Popelka, A Sosna
Acta Chir Orthop Traumatol Cech. 1996; 63(1):40-43
The authors present in their case-history two cases when an osseous bridge developed in the space of the distal femur and distal radius. After destruction of the osseous bridge they used for interposition part of a silastic prosthesis of the trapezius to prevent a relapse of the osseous bridge. Although the results are only short-term, in both patients it proved possible by means of the silastic interposition to achieve satisfactory results and to prevent a new adhesion in the affected growth space. Key words: osseous bridge, epiphyseal fractures.
[Contemporary view on the treatment of deformities of the spine.].
O Vlach
Acta Chir Orthop Traumatol Cech. 1996; 63(1):44-46
The author draws attention to errors which are still encountered in the treatment of deformities of the spine, and to recent findings. In conservative as well as surgical treatment of scolioses little attention is paid to curvatures in the sagittal plane. Thoracic hypokyphosis is relatively frequent, it has a significant impact on pulmonary functions, it reduces the effectiveness of orthosis and plays also a significant role in the indication for surgery. Conservative treatment involves the wearing of an orthosis, physical exercise and other rehabilitation procedures do not affect the curvature. Correct indication, effective orthosis and a correct...
[History of surgical approaches into spinal canal.].
F Helcl
Acta Chir Orthop Traumatol Cech. 1996; 63(1):47-50
The article brings the historical survey of various surgical approaches into the spinal canal from the point of view of contemporary neurosurgery. The author comes out from the retrieval of English and American literature obtained from the University library during his study stay in Zürich. Some sources from the article are not available for Czech readers. Historical contribution of the famous Czech surgeon Maydl working at the end of last century in Vienna is emphasized. Key words: history, surgical approaches, spinal canal.
