Biomechanical reaction of bone to intramedullary and extramedullary load bearers - the important contact.

S M Perren

Acta Chir Orthop Traumatol Cech. 1996; 63(3):132-138

Bone reacts in a wide range of ways to the changes in its local environment imposed by implant surgery. The advantages and disadvantages of implants must be carefully weighed up, and the first step is therefore to consider the functions of implants and the requirements that they must meet. The contact zone between implant and bone is then discussed. Contact surfaces and their tendency to induce necrosis are nowadays at the forefront of interest in our studies in the realm of biomechanics, i. e., the study of the biological reaction of bone to alterations in mechanical conditions. The salient topics are: avoidance of early temporary osteoporosis, and...

[King's Classification of Thoracic Idiopathic Curves.].

O Vlach

Acta Chir Orthop Traumatol Cech. 1996; 63(3):139-145

In 1983 King et al. published a retrospective study of 405 patients with an idiopathic curve located in the thoracic spine, operated by Harrington's method. On the pre-operative X-rays the authors assessed the flexibility index, using a central sacral line, the stable vertebra and compared the findings with the postoperative ones. Based on the assembled data they divided the curves into five groups and elaborated basic rules for assessment of the extent of fusion. This publication had a worldwide response and was mentioned in our professional journals (17). The submitted paper provides more detailed information on this classification and the author...

[Non-cemented Ceramic Acetabular Component.].

A Sosna, J Beznoska, D Jahoda, I Landor

Acta Chir Orthop Traumatol Cech. 1996; 63(3):146-151

In 1989-1992 the authors used at the Orthopaedic Clinic, First Medical Faculty, Charles University Prague 34 ceramic conical non-cemented sockets. They used 12 original sockets type AUTOPHOR Osteo Co. and 22 local sockets manufactured by Dias Co. On evaluation of 25 patients, i. e. 26 hip joints, when using Harris score, the mean increment was 56 points, the mean follow up period two years. For evaluation the authors used AP projections as well as tangential pictures focused on the threads which make a more exact evaluation of the integration of the socket possible. The authors found that the radi-olucent line is lacking in 50 % patients, in the remainder...

[Hemiarthroplasty of the knee joint, possibilities and limitations of its use.].

P Sponer, K Urban

Acta Chir Orthop Traumatol Cech. 1996; 63(3):152-157

In the submitted paper the authors evaluate the results of hemiarthroplasty of the knee joint at the Orthopaedic Clinic in Hradec Králové in 1985-1992. To a total of 40 patients 53 sledge-shaped endoprostheses of Link Co. were administered. Twenty-five patients with 30 endoprostheses and a mean follow up period of 5.6 years were checked. For evaluation criteria published by Joseph and Kaufman were used. In 60 % the results were excellent or good, in 30 % satisfactory and in 10 % unsatisfactory. Re-operations were performed in two patients. On the postoperative X-rays the authors examined radiological signs of loosening of the endoprostheses. They did...

[Complications of hinged endoprostheses of the knee joint, endomodell link.].

V Stedrý, J Pilnácek

Acta Chir Orthop Traumatol Cech. 1996; 63(3):158-161

The authors implanted in 1985-1992 25 hinged endoprostheses of the knee joint, type Endomodell Link. It is a type of endoprosthesis which belongs into the group of so-called modified hinged prostheses. In addition to flexion and extension the design of the implant permits also 15 degree axial rotation. Indication for operation were deformities of the knee joint in 21 cases, re-operation in two knee joints and bony ankylosis associated with rheumatoid arthritis in two knee joints. The mean age of the patients was 64 years. In two patients infectious complications developed and the implant had to be removed. The authors did not record in any of the patients...

[Resection in the region of the shoulder joint in musculoskeletal tumours.].

P Janícek, O Jelínek, J Cerný

Acta Chir Orthop Traumatol Cech. 1996; 63(3):162-167

The authors evaluated the results of resections of musculoskeletal tumours in the area of the shoulder, operated in 1975-1993 at the First Orthopaedic Department in Brno. For evaluation they used Malawer's six grade classification and differentiated indications for different types of resections. The extent of resection depended on the biological nature of the tumour, its size and site. Key words: musculoskeletal tumours, schoulder girdle, resection.

[Initial experience with the treatment of diaphyseal fractures of the leg by an intramedullary nail.].

B Soukup, T Malkus

Acta Chir Orthop Traumatol Cech. 1996; 63(3):168-173

The authors present an account on their clinical experience with the treatment of diaphyseal fractures of the leg using an intramedullary nail not drilled in advance. During the period between May 1994 and August 1995 they treated at the Orthopaedic Clinic of IPVZ in Prague Bulovka hospital a total of 28 patients, 22 men and 6 women, mean age 36 years, follow up period six months. The final clinical evaluation comprised 26 patients with 27 fractures. In 93% the authors recorded complete cure of the fracture of the leg, on average within five months after operation. The group of patients comprised 7 (25%) patients with multiple injuries and 5 (17%)...

[Treatment of polytraumatized patients: our experience.].

J Michek, P Zelnícek, J Vrastyák, M Sutorý

Acta Chir Orthop Traumatol Cech. 1996; 63(3):174-176

Authors evaluate their experience in treatment of patients with polytrauma during last 10 years. They use the 5 level classification applicated particularly in German and Austrian traumatologic centres. Authors initiated in their workplace the computer assessment ISS - Injury Severity Score, Apache II -Acute Physiology and Chronic Health Evaluation II. The care for polytrauma requires the highly qualified individual approach and should be subordinated to threatening or developed complications. The question of timing of emergent operations in polytraumatized patients is solved that was one of research projects of this institution, too. The reconstructional...

[Arthroscopy of the ankle.].

B Delej

Acta Chir Orthop Traumatol Cech. 1996; 63(3):177-182

The author describes in his paper in detail the method of arthroscopy of the ankle. He analyzes indications, contraindications, the technique of the operation and complications of arthroscopy of the ankle. He evaluates a group of 22 patients operated in 1995 and compares the results with those achieved abroad. The author presents his own suggestions for the surgical protocol during arthroscopy of the ankle and a non-invasive arthrodistractor of his own make. Key words: arthroscopy of the ankle, impingement syndrome of the ankle, surgical arthroscopy of the ankle, arthrodistractor.

[Treatment of Acromioclavicular Dislocations by Weaver-Dunn's Operation.].

M Kitka, V Blasko, J Károlyi, S Novotný

Acta Chir Orthop Traumatol Cech. 1996; 63(3):183-185

The authors present an account of a group of 52 patients subjected to Weaver-Dunn's operation on account of acromioclavicular dislocation grade III. They started to use this method in 1988. The authors emphasize that it is an elastic fixation of the clavicle without the use of osteosynthetic material. It is a simple operation with a relatively short period of work incapacity. Based on the described group of patients the authors provide evidence that they achieved very good results with Weaver-Dunn's operation. Key words: acromioclavicular dislocation, Weaver-Dunn's operation.
