[Prosthetic arthroplasty in the region of the knee for musculoskeletal tumours].

The authors evaluated a group of 23 patients (13 women and 10 men) with resection of the distal end of the femur or proximal part of the tibia for musculoskeletal tumours in the knee joint region. Resected bone was replaced by individually made tumorous total endoprostheses. Most frequently the authors operated on account of osteosarcoma (13 cases) and chondrosarcoma (5 cases). The proximal tibia was replaced in 11 patients, the distal femur in 12 patients. Endoprosthesis of 5 types and from 4 firms were used. Johnson and Johnson implants and Poldi type were most frequent. For the investigation pig chondrocytes were used which were cultivated in a medium containing cartilaginous collagen and aggregan. The substances used were identified by electrophoresis, chromatography and electron microscopy. Various conditions for cultivating chondrocytes were tested - a) a medium containing cartilaginous collagens only, b) with addition of aggregan, c) with addition of the tripeptide GHK, d) with addition of the tripeptide and aggregan. The published paper pertains to a group of fullgrown minipigs where under anaesthesia with Narcamone after premedication with Rometar cartilage samples from the medial condyle of the femur in the area of the femoropatellar joint were taken. The authors investigated possibilities and ways of reconstruction of the talar joint in poorly healed fractures associated with a lesion of the tibiofibular syndesmosis. The anatomical investigation revealed that for extension of the fork of the talus the proximal shift of the ankle is most important (9, 10) . The authors submitted the clinical results of an investigation of 43 patients where in 1986 - 1995 at the Orthopaedic Clinic of the Institute for Postgraduate Training in Prague, Bulovka osteosynthesis by means of a corrugated splint was implemented. The authors describe the diagnostic and therapeutic procedure in diseases of the shoulder joint with regard to arthroscopic possibilities, assembled over a five-year period. Surgery of the shoulder joint accounts at the Clinic for 4 % of all surgical operations. Persisting or newly developed sequelae of spinal injuries can be encountered after conservative as well as surgical treatment. On the spine it is a deformity, most frequently a pathological kyphosis, instability or possibly stenosis of the spinal canal. All these factors can evoke pain and lead secondarily to progression of an already present nervous affection or to its late development. [Chronic tibialis anterior syndrome.].

Chronic tibialis anterior syndrome affects most frequently adolescent sportsmen. It is manifested by pain on the anterior side of the leg and sometimes also by claudications after burdening. The cause is hypertrophy of the m. tibialis anterior which leads to increased subfascial pressure and thus development of the syndrome. The diagnosis is frequently incorrect and the condition is treated as periostitis or enthesopathy. As the cause is increased subfascial pressure after burdening, causal treatment is fasciotomy of the anterior compartment of the leg. [Double serious complication of the surgical treatment of a female patient with scoliosis (case report).].

The authors present the case-history of a female patient who developed during operation of a thoracolumbal scoliotic curve haemorrhagic shock with circulatory arrest due to massive haemorrhage from the spongiosa. After more than six weeks after admission to hospital she developed tense hameothorax from the injured diaphragm caused by the edge of the osteomotized rib of the concavity. The condition was treated by leftesided posterolateral thoracotomy. The authors draw attention to some special features associated with treatment of deformities of the spine.
