This paper evaluates the clinical, radiological and histomorphologic effect of neoadjuvant chemotherapy in 37 patients with II B osteosarcomas. The results are compared according to the type of surgery, local recurrence and lung metastasis at a minimal follow up of five years in patients who underwent surgery at the orthopedic clinic in Prague Bulovka in 1982 - 1992. All patients continued with adjuvant chemotherapy after surgery. Bad clinical response (increase of circumference, decrease in range of motion and increase of pain) correlated with the bad histologic response evaluated according to the COSS criteria. Accurate evaluation of a complete radiological...
[Non-cemented CLS Spotorno Socket and Cemented Polyethylene Socket in Revised Replacements of the Hip Joint.].
Z Rozkydal, P Janícek, Z Smíd
Acta Chir Orthop Traumatol Cech. 1997; 64(5):275-281
The objective of the presented work was to evaluate the results of revised replacements of the hip joint using non-cemented CLS Spotorno sockets and cemented polyethylene sockets of 1992 - 1993. The baseline situation was identical: an aseptically loosened polyethylene socket, intact pillars and preserved acetabular margins. In the group of CLS Spotorno sockets there were 56 patients, 52 were evaluated after an average period of 42 months following revision. In the CLS Spotorno group the mean Harris hip score was at the end of the study 84.6 points. The authors recorded complete osteointegration of the socket in 45 instances. There were 7 sockets with...
[Long-term Results of TEP of the Hip Joint Poldi.].
V Stedrý, P Hajný
Acta Chir Orthop Traumatol Cech. 1997; 64(5):282-291
The authors evaluate a group of 192 patients operated in 1983 at the Orthopaedic Clinic of the Institute for Postgraduate Medical Training to whom 196 cemented total endoprostheses of the hip joint type Poldi were implanted. Of these 53.1 % patients were operated on account of primary arthritis, 29.1 % on account of postdysplastic arthritis of the hip joint. In four patients conversion of an osseous ankylosis to an endoprosthesis was performed. In 59.7 % a medium Poldi socket was used, in 36.5 % a small Poldi socket and in 13.8 % a large Poldi socket. For fixation in all patients Palacos cement with first generation technique was used. In seven patients...
[Diagnostic and therapeutic procedures in intraabdominal injuries.].
F Vyhnánek, L Novák, J Strnad
Acta Chir Orthop Traumatol Cech. 1997; 64(5):292-295
The authors evaluated the diagnostic and therapeutic procedure in 25 patients with blunt and in 12 with penetrating abdominal injuries hospitalized between Jan. 1, 1995 and Aug. 30, 1997 at the intensive care unit of the Surgical Clinic Prague 10. The diagnostic algorithm included in addition to the basic clinical examination also computed tomography. The most frequently injured organ in blunt injuries was the spleen (12 times) and liver (11 times). Twenty-three patients were operated. In casualties with penetrating injuries there were 8 with piercing wounds and 4 with shotgun wounds. The most frequently injured organ was the small intestine (4 times)....
[The role of osteotomy in the treatment of pseudoarthroses.].
J Pilnácek, B Soukup, I Cechová, P Závitkovský
Acta Chir Orthop Traumatol Cech. 1997; 64(5):296-301
Osteotomies in the area of pseudoarthroses ensue from the necessity to correct biomechanical conditions at these sites. Depending on the site of osteotomy, we speak of intrafocal osteotomies, i. e. wedge-shaped resection (total or partial) of the pseudoarthrosis. The second type of osteotomy is transfocal osteotomy which is normal to the longitudinal axis of each fragment. The third type is parafocal osteotomy which is several centimeters from the pseudoarthrosis and the correction of biomechanical conditions is implemented in this osteotomy. The implementation of osteotomy markedly facilitates the procedure and shortens the time of operation. Healing...
[Osteosynthesis by Unreamed Interlocking Nail UTN of the Tibial Shaft Fractures Caused by High Energy Injuries.].
T Malkus, B Soukup
Acta Chir Orthop Traumatol Cech. 1997; 64(5):302-309
The authors present their own two-year experience with osteosynthesis of 54 fractures of the diaphysis of the leg in 51 patients, using a UTN nail without predrilling, at the Orthopaedic Clinic of the Institute for Postgraduate Medical Training in Prague Bulovka Hospital. After a one-year interval they included in the group 28 complicated fractures of the leg in 26 patients where the fractures were caused by a high energy impact. In nine instances (34.6 %) the fracture was in a polytraumatized patient and in eight patients (50,8 %) with multiple injuries. There were 14.3 % grade 1 open fractures (four fractures) and 21.4 % (four fractures) were grade...
[Double intertrochanteric distraction osteotomy.].
J Vojtassák, T Kubík
Acta Chir Orthop Traumatol Cech. 1997; 64(5):310-315
The authors present four case-histories pertaining to double intertrochanteric distraction osteotomy of the proximal portion of the femur. In two female patients the result was excellent, in one female patient a 130 degrees splint broke and in another the splint was bent and the splints had to be replaced. All patients are satisfied with the operation because the extremity was prolonged on average by 2 cm. The authors objective is to demonstrate, based on their own practice, the static and biomechanical demands laid not only on bone but also on the metal fixation material (splint) in double distraction osteotomy of the proximal femur. Based on these...
