D Pokorný, A Sosna, I Landor, D Jahoda

Acta Chir Orthop Traumatol Cech. 1998; 65(3):133-139

Authors of the article present the results of TKR by LCS cementless prosthesis manufactured by DePuy.This system is aimed at reducing stress concentration between femoral and tibial components in order to minimise the polyethylene wear. This is achieved by the increase of constraint between polyethylene insert and femoral component and at the same time by facilitating the defined motion between the insert and the metal tibial component by the system of mobile bearings. In the period 1992 through 1997 the implant was used at 1 st Orthopaedic Clinic of 1 st Medical Faculty, Charles University in Prague as a joint replacement in 63 cases in 56 patients....

[BAS-0 Bioactive Glass-ceramic as a Bony Tissue Replacement (Clinical Experience after a Longterm Interval after Application).].

P Sponer, K Urban, C Povýsil

Acta Chir Orthop Traumatol Cech. 1998; 65(3):141-152

The authors present evaluation of the application of BAS-0 bioactive glass-ceramics at the Orthopaedic Clinic in Hradec Králové in the period 1990-1997. Bioactive glass-ceramics was implanted in total in 106 patients of which in 38 patients during THR revision surgery and in 20 patients with bone infection. Of 48 patients with bioactive glass-ceramics used as a bony tissue replacement in filling the defects during operations of benign tumours or tumours of similar affections and in the surgical treatment of fractures of tibial plateau, 31 were followed up for the period ranging between 3 months to 6 years (average 3,2 years). In all patients followed...

[Limited indication for percutaneous lumbal nucleotomies.].

M Weissinger, C Helmreich, G Eiserle

Acta Chir Orthop Traumatol Cech. 1998; 65(3):141-144

The aim of our retrospective study is to show how very circumspect we are to use this method. The clinical symptoms of our patients were lumbalgia or lumboischialgia with or without neurological deficiencies. All these patients were resistent to conservative therapies. We accepted for operation only patients who suffered from protrusions and types of prolaps without perforation of the posterior longitudinal ligament. 179 operations on the vertebral discs were performed in our department between 01. 01. 1992 until 31.12.1997. Of this number 165 (92 %) were conservative nucleotomies (63 females and 102 males) and 14 patients (8 %) hat percutane nucleotomies...

[Conversion of Arthrodesis and Ankylosis of the Hip Joint to THR.].

A Sosna, D Pokorný

Acta Chir Orthop Traumatol Cech. 1998; 65(3):153-162

Conversion of arthrodesis or ankylosis of the hip joint toTHR is a very difficult operation from the technical viewpoint. However, with a deliberate indication it may conduce to a significant improvement of the quality of life of the patient. Indications for such operation are the following: arthrodesis in malposition, increasing lowback pain, difficulties with personal hygiene and problems in the sexual life, instability of knee or its deformity given by the incorrect position of the hip joint. Contraindication is florid osteomyelitis, serious damage of muscles in the region of the hip joint, serious neurological affection and myopathy. We do not...

[Changes in Roughness of Exposed Head of POLDI Femoral Component.].

V Stedrý, L Joska, V Chmelík, F Dvorák

Acta Chir Orthop Traumatol Cech. 1998; 65(3):163-164

The authors have examined the roughness of the heads of four exposed all-metal cemented femoral components of Poldi type using Talysurf 6 device. The results achieved were compared with the roughness of the not yet used head of Poldi femoral component. Both in the exposed and not used heads the Ra factor lower than the value reported in production documentation of the manufacturer. Even in a longterm exposition (17 years) there occurred no changes in the roughness of the metal head of the femoral component. Therefore it is possible in revision surgeries due to aseptic loosening of the acetabular component, to replace only the cup and leave the original...

[Evaluation of replantations with regard to duration of treatment and sick-leave.].

A Scerbáková, G Vasko, Z Sidor

Acta Chir Orthop Traumatol Cech. 1998; 65(3):165-166

The authors compare results of replantations performed at the Orthopaedic Clinic of the Medical Faculty, Safarik University in Kosice, Replantation Centres in Austria (which are part of traumatological hospitals) and results of replantations at the Clinic of Plastic and Aesthetic Surgery in Brno. They base their evaluation on the number of the survival of replants, period of treatment and sickleave. In the course of 7 years (1991 through 1997) there were 157 patients operated on at the Orthopaedic Clinic of the Medical Faculty, Safarik University in Kosice for replantation and revascularization. The success rate of replantations has in the recent years...

[Prophylactic osteotomy of pelvis in dysplastic acetabulum.].

M Richtr, A Sosna

Acta Chir Orthop Traumatol Cech. 1998; 65(3):167-175

Acetabular dysplasia and anatomical changes of pelvis accompanied with gracility of iliac bone pose a problem for a reliable fixation of acetabular component. In extreme cases the bone stock is not sufficient to accommodate the smallest size of the cup. Sometimes even reaming of the bed down to lamina interna is not sufficient for an adequate covering of the implant. Consequently, weakening of the bottom of acetabulum may be not only one of the causes of the failure of the implant but it makes difficult also the following revision surgeries. Therefore in acetabular dysplasia the authors recommend acetabuloplasty of the bottom of acetabulum preserving...

[Our Experience in the Use of BAS-0 Bioactive Glass-ceramic in Orthoses in the Region of the Ankle.].

V Smetana, A Schejbalová, J Mrácek

Acta Chir Orthop Traumatol Cech. 1998; 65(3):176-179

Extra-articular arthrodesis after Grice is indicated for the treatment of serious pes planovalgus or equinovalgus after the stabilisation of muscular relations in patients with infantile cerebral palsy. The surgical technique itself inspired the authors to use glass-ceramic blocks BAS-0 which were manufactured in co-operation with LASACK. In the period of 1995-1996 they used the glass-ceramic blocks 15times in 14 patients (1 patient operated bilaterally). Thirteen times they used glass-ceramic block in extra-articular arthrodesis, of which in 9 cases in combination with the Young technique, in two cases as part of subtalar arthrodeses. The criterion...

[Dynamics of cytokines in blood and liquor at the spinal injured patients.].

P Visna, P Wendsche, P Svoboda, I Kantorová

Acta Chir Orthop Traumatol Cech. 1998; 65(3):180-183

Authors followed the increase of cytokines concentration in blood and liquor at spine injured patients. The degree of the neurological damage was classified according to Frankel scale. Mediators: IL-1, IL-2, IL-6, TNF-alfa, Cortisol were investigated. No significant response of IL-1, IL-2, TNF-alfa to the spinal injury was documented. The Cortisol levels in blood and liquor reached vallues of 100 pg/ml. But no statistical correlation between the Cortisol level and various degrees of spinal neurological damage was identified. The increase in Cortisol concentration can be explained by the stress reaction of the organism. The IL-6 blood and liquor concentrations...

[Primary intraosseous liposarcoma - case report.].

J Kordos, F Makai, S Galbavý, J Paukovic, A Svec

Acta Chir Orthop Traumatol Cech. 1998; 65(3):184-186

Primary intraosseous liposarcoma belongs to exceptional tumors rarely found on the sceleton. There are only several tens of cases described in literature and they are published as case reports. Because there is not enough experience, especially with diagnostics of this illness, it happens that it is considered as malignant fibrous histiocytoma, malignant mesenchymoma or with secondary infiltration of liposarcoma of soft tissue into the bone. Authors tried (in correlation with the clinical outcome studies of macroscopic samples with detailed histologic examination) to overcome diagnostic uncertainities of this case. Key words: primary intraosseous liposarcoma,...
