[Postoperative sciatic nerve palsy after hip joint surgery.].

A Sosna, D Pokorný, D Veigl, D Jahoda

Acta Chir Orthop Traumatol Cech. 1998; 65(6):331-335

Postoperative impairment of the function after surgery in the region of hip joint has always a strong negative impact on the final outcome of the surgery. In most patient neurological functions are fully restored, residual impairment affects only a smaller part of them. Specialist literature most frequently deals with the development of lesion during THR. The risk of injury of the nerve is significantly higher in revision surgery relating toTHR reimplantation or inTHR in a changed anatomical region. The cause which leads to the impairment of the sciatic nerve is considered direct injury of the nerve during the surgery, ischemization of the nerve tissue,...

[Anatomic variability of the relation of pelvitrochanteric muscles and sciatic nerve.].

D Pokorný, A Sosna, P Veigl, D Jahoda

Acta Chir Orthop Traumatol Cech. 1998; 65(6):336-339

The article proceeds from the preceding work published in this journal under the title "Impairment of the function of the sciatic nerve after the surgery in the region of hip joint" It studies a hypothesis for another potential cause of the impairment of the function of the sciatic nerve, i.e. overdistension of the sciatic nerve in the region of pelvitrochanteric muscles after their tenotomy. In order to verify this hypothesis the authors studied 51 cadavers. They describe first of all mutual relation of m. piriformis, m. triceps coxae and the sciatic nerve. The findings were always classified according to one of the 6 types after Beaton. The authors...

[External fixation in fractures of distal radius.].

Z Klézl, J Fousek, J Cyprich

Acta Chir Orthop Traumatol Cech. 1998; 65(6):340-344

On a group of 30 patients authors present the results they have achieved in the treatment of fractures of distal radius by external fixation. They deal with indications to the treatment of this injury by external fixation and describe in detail the method of such treatment. They discuss the pitfalls of this treatment, potential complications and adequacy of individual types of external fixators. They point out the discrepancy in the evaluation of objective achievements and subjective evaluation of patients. In total they evaluate the results as good and recommend a more consistent diagnostic and therapeutic connsidera-tion of the treatment of these...

[A new minimally invasive approach to interior lumbal spine fusion.].

J S Thalgott, Z Klézl, T Kossmann, J Guiffre

Acta Chir Orthop Traumatol Cech. 1998; 65(6):345-348

The authors are presenting results on minimally-invasive laparoscopic approach to obtain anterior fusion of the lumbar spine on group of 62 patients. 12 patients had to have a conversion to open procedure. 50 patients were included in the follow up and evaluated in average 22 months after surgery. The advantages of the approach were documented by good overall rating of the procedure by the patients themselves, low blood loss, short hospital stay and high fusion rate. More than 50 % of patients had previously at least one spine surgery. The endoscopic retroperitoneal gasless approach (BERG) is very convenient as it enables surgery without gas insuflation...

[Congenital posteromedial angulation of tibia and fibula.].

J Poul

Acta Chir Orthop Traumatol Cech. 1998; 65(6):349-352

The author has based his article on all available literature dealing with a rare congenital clinical unit "congenital posteromedial angulation of tibia and fibula". Congenital kyphoscoliosis of tibia is a synonym for the same disease. In contrast to other angulations of the tibia and fibula the above mentioned clinical unit is characterized by a spontaneous remodeling of the bone deformity. Persisting is only the shortening of tibia. The author describes in the study his own follow-up of four cases. In one case it was necessary to correct valgus deformity of tibia in 8th month of age. Angulation in all four children spontaneously corrected itself before...

[Evaluation of Stages of Legg-Calvé-Perthes Disease Using Conway's Scintigraphic Classification.].

D Chroustová, V Dzupa, P Dungl, J Chomiak

Acta Chir Orthop Traumatol Cech. 1998; 65(6):353-358

Legg-Calvé-Perthes disease (LCPD) is considered as avascular necrosis of the proximal femoral epiphysis, that can result in a secondary osteoarthritis. Bone scintigraphy using 99mTc phosphonate complexes is a suitable method for study of changes of LCPD. The authors present a group of 59 children (44 boys, 15 girls), with clinical suspicion of LCPD or with clear diagnosis of LCPD according to X-ray examination, who were examined using scintigraphy. The patological scintigraphic images of the proximal femoral epiphysis are evaluated according to Conway's classification. The correlation of pinhole imaging of the hip X-ray examination is neccessery for...

Acta Chir Orthop Traumatol Cech. 1998; 65(6):359-364

Authors evaluate their own experience in the treatment of constricting annular grooves of limbs, especially the hand, which in the total number of treated congenital anomalies of limbs account for 172 cases (17 %). Hands were affected in 152 cases (88,4 %), legs in 20 cases (11,6 %). The authors point out a great variety of the affection by Constricting annular grooves and ensuing from it a strictly individual rational surgical technique. As the most important they consider adequate followup and repeated assessment of the condition of the malformed limb until adult age. They state that even with amputation of part of fingers the patients are able to...

[Topical issue of the injury of liver.].

J Zelenák, M Hutan, K Kálig, V Poticný

Acta Chir Orthop Traumatol Cech. 1998; 65(6):365-369

The authors analyse their own 10-year clinical material (1 August 1987-31 July 1997). In the course of this period they operated on 40 patients for the injury of liver. The indications were persisting intra-abdominal bleeding, multiple injuries of organs in the abdominal cavity, simultaneous injuries of diaphragm and gunshot wounds of abdomen. Injuries were classsified according to AAST, the range of organ injury - liver. The treatment of liver was determined by the extent of the injury. In their surgical strategy the authors prefer suture of hepar by collagenous stripes (Parenchymaset, Braun) with sufficient drainage, but they also used resection...

[Injuries of cervical spine: part I - c0-c2.].

L Tóth, M Trnovský

Acta Chir Orthop Traumatol Cech. 1998; 65(6):370-380

Authors have evaluated 32 patients with injuries in the region of C0-C2 treated in the period of 1990-96. By the evaluation of 11 patients operated on and 21 patients treated conservatively they present a brief survey of potential injuries in this region, of the principles of diagnosis, suitability of individual examination methods, they evaluate the principles of reduction and fixation and determine indications and contraindications of halo traction and halo vests. They provide a guideline for the determination of primary and secondary instability with definition of limits of conservative therapy including the specification of its principles and finally...
