[Valgus Osteotomy in the Treatment of the Ischemic Necrosis of Femoral Head in Children: Part II - Clinical Evaluation of the Group.].
J Vávra, O Cech, M Zídka
Acta Chir Orthop Traumatol Cech. 2000; 67(2):77-87
The authors present long-term results of the surgical treatment of ischemic necrosis of proximal femur resulting from the treatment of the developmental dysplasia. They used their own technique of intertrochanteric valgus osteotomy in 77 hip joints. The surgical technique was published in the first part of the work. It consists in valgus osteotomy which lengthens femoral neck and, consequently also the whole limb. At the same time a fusion of the physis of the greater trochanter was performed and the pathological antetorsion of the femoral neck corrected. Osteotomy was fixed with a special valgus angled blade plate. In cases where dysplastic acetabulum...
[Medium-term Experience with Zweymüller's Bicon Plus Cup in the Treatment of Postdysplastic Osteoarthritits of the Hip Joint.].
P Dungl, V Stedrý, P Hajný
Acta Chir Orthop Traumatol Cech. 2000; 67(2):88-92
The authors retrospectively evaluate a group of 28 hip joints in which in 1995 and 1996 total hip replacement was performed for developmental dysplasia of the hip joint. They included in the study only patients with dysplasia of types B, C and D after Eftekhar. By means of the method after Ranawat and Pagnan they determine the ideal centre of the rotation of the hip join and the so called TAR-true acetabular region. They checked the patients in 1999 and evaluated subjective complaints, objective findings according to Harris Hip Score and x-ray findings on a radiograph of the pelvis and on the radiograph showing a 45 degrees rotation (the Judet view)....
[Magerls technique of c1-2 fixation.].
J Stulík, M Krbec
Acta Chir Orthop Traumatol Cech. 2000; 67(2):93-99
Magerl technique of transarticular fixation of C1-2 by screws is one of the possibilities of the antlantoaxial fusion. For the first time it was published by Magerl and Seemann in 1987 but the first author had been performing it already since 1979. The principle of the method consists in the placement of two screws from the dorsal approach through atlantoaxial joints in the sagittal plane. The main indication for the Magerl's C1-2 fixation is an acute or chronic instability and a painful osteoarthritis of atlantoaxial joints. Contraindicated is this method only in massive destruction of the lateral mass of atlas and in an anomalous course of the vertebral...
[The First Results of 360 degrees Fusion and Video-assisted Minimally Invasive Retroperitoneal Approach to the Lumbar Spine.].
Z Klézl, J Svárovský, J Fousek
Acta Chir Orthop Traumatol Cech. 2000; 67(2):100-104
The authors present short-term results of a 360 degrees fusion together with the experience in their own modification of minimally invasive retroperitoneal approach to the lumbar spine. In the course of 1998 and 1999 they operated on the total number of 21 patients using this approach. The anterior approach was considered as part of the combined operation on the lumbar spine which was indicated for degenerative, traumatic, tumorous and iatrogenic instability of the lumbar spine. They evaluated blood loss, duration of the surgery, duration of image intensifier exposure, restoration of the peristalsis, satisfaction of the patient with the operation by...
[Hallux valgus - indication for surgery.].
F Hudecek, T Tomás
Acta Chir Orthop Traumatol Cech. 2000; 67(2):105-108
The authors assessed the outcomes of the operation of hallux valgus in 73 patients operated on at the Ist Orthopaedic Clinic in Brno in the period between 1991 and 1994.The operations were either performed on soft tissues (Schede, McBride) or they were distal osteotomies (Mitchell, chevron osteotomy). The authors present a list of criteria important for the choice of either method of surgery. In their retrospective study they monitor the satisfaction of patients in the interval between 54 and 76 months and compare the satisfaction of patients in the group where all indication criteria were met with the satisfaction of patients in a group where some...
[Posttraumatic avascular necrosis of the scaphoid bone, our arthroplasty modification after dwayer.].
M Lutonský, M Holík
Acta Chir Orthop Traumatol Cech. 2000; 67(2):109-112
In the introduction the authors present this unit from the viewpoint of its incidence. With regard to its etiology and pathoanatomy its development is compared to the aseptic necrosis of femoral head. A scheme illustrates the basic surgical solutions of this condition. The modified method after Dwayer was performed at the authors' department in three cases since 1995. The authors emphasize the principle not to remove a greater portion of the bone than one quarter of the total size. Discussion deals with the Preiser disease which represents an idiopathic aseptic necrosis of the scaphoid bone. Apart from the outcomes of standard surgical methods the...
[Zweymüller's Endoprosthesis in Total Hip Arthroplasty for Postdysplastic Osteoarthritis.].
K Koudela, J Ferda
Acta Chir Orthop Traumatol Cech. 2000; 67(2):113-120
The authors evaluated a group of 224 patients, 148 females and 76 males, the average age 56 (range 39-75) in which total hip arthroplasty was performed for postdysplastic osteoarthritis by means of Zweymüller's cementless total endoprosthesis. In the period from September 1995 until December 1998 in total 236 hip joints were operated on, of which 12 bilaterally. According to the Crowe classification 76 of 236 hip joints were of Degree I, 149 of Degree II, 11 of Degree III and 0 of Degree IV. Different types of osteotomies were performed previously in 16 hip joints. During preoperative planning 3D CT scanning and multiplanar reconstruction was used...
[Experience in the treatment of infections of the total knee arthroplasty.].
P Vavrík, I Landor, D Jahoda
Acta Chir Orthop Traumatol Cech. 2000; 67(2):121-127
This retrospective study evaluates medium-term results of a two-phase treatment of infections of total knee arthroplasty. In the period of 1990-1997 thirty-three infected total knee replacements were treated (once bilaterally) in 32 patients (21 females and 11 males). In 20 cases the basic diagnosis was osteoarthritis and in 13 cases rheumatoid arthritis. The results were evaluated in the interval of 34-82 months after the revision surgery. The success of the therapy was evaluated on the basis of the fulfilment of the following criteria: a functional implant, the period of post-operative follow-up longer than 24 months, the patient subjectively without...
[Treatment of acromioclavicular dislocation after bosworth.].
J Drímal, P Gémes, J Masár, D Zibolen
Acta Chir Orthop Traumatol Cech. 2000; 67(2):128-132
The authors present a group of 60 acromioclavicular dislocations of Tossy III type in 59 patients whom they operated on in the period of 1992-1996. Within the use of a variety of methods of the surgery of acromioclavicular dislocation they achieved the best results by a timely suture of the coracoclavicular ligament and stabilization of the clavicula by a Bosworth screw. They have been using this method most frequently since 1990 as when performed in time and technically correctly it is very efficient despite its certain drawbacks. The authors emphasize mainly technical aspects of a timely acromioclavicular stabilization by a cancellous screw. In the...

[Primary retrofascial abscess in the iliopsoas muscle caused by salmonella enteritidis.].
Z Matejovský Ml, J Matejovská, J Havlík, H Zídková, Z Serclová, M Reisingerová
Acta Chir Orthop Traumatol Cech. 2000; 67(2):138-142
This case report and review of literature demonstrates a rare entity of a primary retrofascial abscess in the iliopsoas muscle caused by Salmonella enteritidis, that progressed into the iliac fossa. It is of importance to the orthopedic surgeon in his differential diagnosis of hip pain and vertebrogenic disorders. It can also resemble other infectious diseases or tumors as it occurs mainly in young or imunosuppressed individuals. The primary and secondary etiology of this disease and it's different names are discussed together with the underlying bacteria. Clinical, laboratory and radiological findings are compared as well as the outcome of different...
[Osteochondroma of the first rib.].
B Steno, L Lehotský, M Lisý
Acta Chir Orthop Traumatol Cech. 2000; 67(2):143-144
Authors describe a rare case of a solitary osteochondroma of the first rib in a 21 years old male. There have been only 3 cases reported in the medical literature so far. In those cases osteochondroma was never the cause of symptomatology from side of the vascular system. Clinical and CT examination definitely showed the cause of symptoms and the extent and localization of the osteochondroma. Complete resection and exstirpation was followed by rapid full recovery. Key words: osteochondroma of the first rib, CT diagnosis.
