[Structural Bone Grafting in THR with Cup for Dysplastic Hip.].

The authors present results of THR with cementless CLS cup and massive structural bone grafting for dysplastic hip in 42 patients at the average 7,3 years after surgery. They used an autogenous graft from the femoral head in all cases. Crowe type of dysplasia: group I. 14 patients, group II. 20, group III. 5 and group IV. 3 patients. Harris hips score has improved from 37 to 79 points.The cup was placed into anatomic hip centre in 14 patients. The authors present their experience with total hip replacement in patients with ankylosing spondylitis. In the period of 1985-1997 they performed in total 66 total hip replacements in 41 patients. Of the total number of the used implants 24 were fully cemented (the average follow-up - 13 years), 3 were hybrid and 39 cementless (the average follow-up - 6 years). In our University department 135 patients were operated and followed up for minimally 2 years postoperatively. Isola system was used in 92 patients, Miami-Moss in 43 patients. The average curve magnitude was preoperatively in Isola group 61,0 degrees , in Miami-Moss 54,8 degrees . Isola was used exclusively posteriorly, Miami Moss also anteriorly (16 cases). Based on the group of 22 patients operated on for a blunt injury of the thorax a diagnostic-therapeutic algorithm was evaluated in the procedure of the treatment of a severe trauma of the thorax. Acute thoracotomy or laparotomy was performed in 17 patients and in 5 of them thoracotomy was indicated only after some time interval. In the patients with acute surgery the indication was a rupture of diaphragm, massive hemotorax in case of lung laceration of bleeding from thoracic wall, rupture of bronchus and an associated injury of intra-abdominal parenchymal organs. The study present the results of arthrolysis of the elbow in 32 patiens operated on during the period of 10 years, i. e. between 1988 and 1997. [Protrusion of Acetabular Cup in THR.].

The authors report on a case of a complete protrusion of the cup of the hip joint into pelvis. Angiographic examination documents a considerable displacement of iliac vessels caused by the pressure of the displaced acetabular cup which poses a great risk of the injury of vessels during the revision surgery. As a solution of this complication the authors used a separate extraperitoneal approach described in literature as an approach to iliac vessels after Leitz allowing a safe removal of the acetabular cup and the ball of the prosthesis. [Isolated dorsal radiocarpal dislocation.].

The authors report a case of isolated dorsal radiocarpal dislocation without intraarticular fracture of the radius. This injury is very rare and only several case reports we can found in literature. Dislocation of the distal radioulnar joint was also present without ulnar styloid fracture.The mechanism of this injury is described exactly according to data of the patient. Closed reduction and plaster cast fixation brought subjectively satisfactory result and objectively only light restriction of movement and grip strength of the injured extremity without signs of instability.
