Less invasive stabilization system (LISS) in the treatment of distal femoral fractures.

Acta Chir Orthop Traumatol Cech. 2003; 70(2):74-82

The treatment of distal femoral fractures has been associated with a high rate of complications for a long time. Although implants and surgical techniques have improved, plate osteosynthesis and intramedullary nailing have been accompanied by a high occurrence of infection, non-union and malalignment. The treatment of soft tissue envelopes using "biological" osteosynthesis and minimally invasive approaches has resulted in a decrease in complication rates and ultimately led to the concept of the less invasive stabilization system (LISS). This is an extramedullary-applied, internal fixator shaped according to the implantation site anatomy, with minimal...

[Bone grafts in hip prosthesis revisions].

M Krbec, J Adler, P Messner

Acta Chir Orthop Traumatol Cech. 2003; 70(2):83-88

PURPOSE OF THE STUDY: We describe the collection, storage and examination of allogenic bone grafts and their use in revision hip arthroplasty, indicated because of acetabular loosening, that involved either an uncemented cup or an augmentation device with a cemented cup according to the extent and nature of acetabular defects.MATERIAL: Allogenic bone grafts, usually long bones, were collected from deceased humans within 12 hours of death. They were processed according to the standards of US tissue banks. Using ELISA, the blood of each donor was serologically examined for antibodies against selected antigens (HIV I, HIV II, syphilis, HbsAg, HCV,...

[Results of lumbar diskectomy].

J Vaculík, L Tóth, M Trnovský

Acta Chir Orthop Traumatol Cech. 2003; 70(2):89-94

PURPOSE OF THE STUDY: We evaluated the outcomes of lumbar discectomies in relation to the presence or absence of degenerative spinal lesions and the surgical treatment used in order to determine the extent to which these factors influence the result of treatment.MATERIAL: Patients who were treated by discectomy in the period from January 1, 1994, to July 7, 1998, were contacted. A total of 132 subjects received the Oswestry questionnaire (in two copies) in February 2002 and 100 of them, 62 men and 38 women, were included in the final evaluation. The average age of the patients at the time of surgery was 42.86 years (range, 15 to 69 years). The...

[Disarticulation of the knee joint].

V Havlícek, P Janícek, I Berka

Acta Chir Orthop Traumatol Cech. 2003; 70(2):95-99

PURPOSE OF THE STUDY: The authors report their experience with exarticulation in the knee joint, describe the relevant surgical procedure, assess the outcomes of different modifications of the surgical treatment and emphasize the advantages of the stump following exarticulation in the knee joint.MATERIAL: The sample included 19 patients, 13 women and 6 men, operated on in the period from 1987 to 2001. One of the women had bilateral surgery. Their average age at the time of surgery was 48 years, the range was 5 to 82 years. In most of the patients, exarticulation in the knee joint was indicated because of a malignant tumor in the crux region that...

[Treatment of calcaneal fractures using the medial approach for open reduction].

L Vanĕcek, T Malkus, P Dungl

Acta Chir Orthop Traumatol Cech. 2003; 70(2):100-107

PURPOSE OF THE STUDY: A comprehensive evaluation of the treatment of calcaneal fractures by open reduction is presented. The long-term results are compared with the relevant literature data.MATERIAL: In the period from 1987 to 2000, 60 displaced fractures of the calcaneus were treated in 51 patients. Of these, 49 patients suffered the fracture by falling from a height, which is generally the most common cause. Only two patients were injured during sports activities. Thirty eight injuries were classified as fractures of the joint depression type and 22 as fractures of the tongue type. The patients had surgery on the day they sustained the injury...

[Fractures of the distal radius. Part 1: statistical evaluation of the patient group].

V Pacovský

Acta Chir Orthop Traumatol Cech. 2003; 70(2):108-111

PURPOSE OF THE STUDY: Distal radial fracture is one of the most frequent skeletal injuries in the human population. Probably because of this common occurrence, it has long been underestimated and treated as a trivial injury, with the resulting 40% poor outcome. The aim of this study was to analyze relevant characteristics, fracture types and subsequent treatment in patients with distal radial fractures in a one-year follow-up, and to suggest procedures for achieving the best functional results.MATERIAL: A total of 383 patients older than 15 years were treated in our department in 1998. The evaluation of treatment outcomes was based on a special...

Sacroiliac joint dislocation in 11 years old boy treated by open reduction and internal fixation.

M Rysavy, M A Khayarin, K Arun

Acta Chir Orthop Traumatol Cech. 2003; 70(2):112-115

Pelvic fractures in children are relatively rare, especially unstable pelvic ring disruptions. Generally, nonoperative treatment has been recommended for children with these injuries because healing is rapid and remodeling extensive in children. Treatment generally is a period of recumbency until the fractures heal, followed by gradual weight bearing. Various methods of operative treatment were advised by some authors. They reported a higher incidence of residual problems after pelvic disruption such as malunion, hemipelvic undergrowth, low back pain and leg length discrepancy. These authors are of opinion, that principles of treatment should not differ...

[Orthopedic treatment of a boy with Larsen's syndrome].

A Schejbalová, T Trc

Acta Chir Orthop Traumatol Cech. 2003; 70(2):116-120

A case of Larsen's syndrome in a boy is presented. The syndrome is characterized by flattened facies, multiple congenital dislocations and foot deformities as a result of connective tissue maldevelopment during gestation. The importance of early intensive conservative therapy is emphasized. In the case reported, conservative therapy led, in the first stage, to the management of subluxation of both hips and improvement in left foot position and in the state of both knees. Subsequently, dislocations of the right and the left knee joint were surgically treated at 16 and 20 months, respectively, by reduction and transfixation with Kirschner's wires according...

[Traumatic spondylolisthesis of L5-S1].

P Barsa, P Buchvald, P Suchomel, R Lukás

Acta Chir Orthop Traumatol Cech. 2003; 70(2):121-125

Pure traumatic bilateral lumbosacral dislocation is a rare injury with just eight cases reported in the literature. This condition occurred also in 36-year-old man, who was struck into the lower back by a falling tree, during a woodcutting, at the moment when he was kneeling and his spine was flexed. Neurological examination showed no signs of spinal nerves injury. X-ray examination of the lumbosacral spine revealed the presence of a 40% anterior dislocation of L5 over S1 with locked facets and multiple fractures of transverse processes. Computer tomography confirmed these findings and also revealed massive medial L5-S1 disc herniation. Surgery performed...
