Acta Chir Orthop Traumatol Cech. 2004; 71(5):265-271
PURPOSE OF THE STUDY: Shoulder arthroplasty in traumatological indications has a considerably worse prognosis than when the indication is osteoarthritis or rheumatoid arthritis. The problem lies in the technique of rotator cuff reconstruction and the restoration of adequate bio-mechanical properties of the shoulder joint. In this study, the difficult reconstruction of four-part fractures of the proximal end of the humerus was improved by the development of a new type of shoulder joint endoprosthesis.MATERIAL AND METHODS: Our endoprosthesis permits fixation of bone fragments of the greater and lesser tubercles to the shaft and to the diaphyseal...

Acta Chir Orthop Traumatol Cech. 2004; 71(5):272-276
PURPOSE OF THE STUDY: To evaluate the long-term results of shoulder arthroplasty in trauma cases, using ProSpon and MMS-M.I.L. novel systems.MATERIAL AND METHODS: The shoulder endorosthesis designed by the authors, and manufactured as two products by ProSpon (Czech Republic) and MIL (France) companies, was used in surgical treatment of 43 cases of acute fractures and 33 cases of post-traumatic conditions. The Constant score was used to evaluate the clinical outcome.RESULT: In patients with acute trauma, the average Constant score was 59 points (range, 23-82). The average maximal elevation was 88 degrees (range, 40-140). No postoperative pain,...

Acta Chir Orthop Traumatol Cech. 2004; 71(5):277-280
INTRODUCTION: Despite advanced diagnostic methods, biopsy of vertebral body remains to be a very important step in treatment of spinal pathologies. Aim of the study was to present experience with mini-open transpedicular biopsy of the vertebral bodies of the thoracolumbar spine in the circumstances of present trend of CT guided needle biopsy preference.MATERIAL AND METHODS: Standard surgical technique was used in obtaining samples in a group of 38 patients during 1996-2001. Indication for biopsy was to identify the etiology of pathological findings on plain films, CT scans or MRI. 21 patients were men and 17 women average age was 59 years (26-76)....
[Palliative Schanz osteotomy for irreducible hip dislocation in adolescent patients with cerebral palsy].
A Schejbalová
Acta Chir Orthop Traumatol Cech. 2004; 71(5):281-287
PURPOSE OF THE STUDY: Palliative Schanz osteotomy is one of the options for treatment of irreducible hip dislocation in adolescent patients with cerebral palsy.MATERIAL: In 1992 to 2002, Schanz osteotomy was indicated on 46 occasions in 27 nonambulatory patients with the quadriplegic form of cerebral palsy aged 9 to 18 years.METHODS: In the postoperative evaluation, emphasis was placed on the clinical presentation, i.e., improved motion of the hip joints and pain alleviation. X-ray examination was carried out at 2 to 6 months after surgery. RESULTS In all patients, the range of motion in the hip increased in abduction or flexion according to...

Acta Chir Orthop Traumatol Cech. 2004; 71(5):288-291
PURPOSE OF THE STUDY: To compare, on the basis of clinical and radiographic findings, the results of non-cemented acetabulum implantation involving the use of a CT-free navigation system with those of implantation without its use.MATERIAL: A total of 50 patients undergoing implantation of a non-cemented acetabulum in the period from April 2002 to September 2003 were evaluated. Twenty-five patients operated on without the navigation system were included in group 1 on a random basis and 25 patients treated with the use of the system constituted group 2.METHODS: Both groups were evaluated on the basis of clinical and X-ray findings. The radiographic...
[Arthroscopy of the elbow joint].

Acta Chir Orthop Traumatol Cech. 2004; 71(5):292-296
PURPOSE OF THE STUDY: The authors evaluate a group of patients who underwent elbow arthroscopy in the period between August 2001 and August 2003. They pay attention to indications, the technique used, surgical diagnoses, postoperative outcomes and complications.MATERIAL: We performed 28 elbow arthroscopies in 26 patients; two patients were operated on both elbow joints. The group comprised 20 men and 6 women between 14 to 73 years, with an average of 44 years. The average follow-up was 18 months (range, 4-28 months).METHODS: We usually operated on patients in a prone position under general anesthesia, with the use of a tourniquet. We used the...
[Results of reinnervation after peripheral nerve repair by a microsurgical technique used in 1996-1998].
K Saur, R Bartos, M Sames
Acta Chir Orthop Traumatol Cech. 2004; 71(5):297-302
PURPOSE OF THE STUDY: The authors present the results of a retrospective study of reinnervation after peripheal nerve repair by a microsurgical technique between 1996 and 1998. This is a development of a previous study by their colleagues.MATERIAL: Between 1996 and 1998, 113 injured peripheral nerves were treated by epineuroperineural suture (Ethilon; thickness, 8/0-10/0) under a microscope. To achieve a homogeneous group, 12 median and 13 ulnar nerves were included and evaluated in this retrospective study. All nerves studied were completely severed at the distal forearm and had sharply cut ends.METHODS: The results of reinnervation were retrospectively...
[Bilateral ankylosing paraarticular ossifications of the hip joint with involvement of the sciatic nerve].
V Stědrý, E Zvěrina, J Chomiak
Acta Chir Orthop Traumatol Cech. 2004; 71(5):303-307
In December 1997 a 48 year-old man was treated by resection of the bilateral paraarticular ankylosing ossifications that developed after 6 weeks of unconsciousness after necrotic pancreatitis. The ossifications on the posterior part of the hip joint involved a highly separated sciatic nerve. There was a bilateral osseous ankylosis of the hip joint and total denervation of sciatic nerve. In April 1999 the ossifications on the right side were removed. The splitted sciatic nerve was in two osseous channels. The head of the femur was vital and immediately following the operation, flexion of the hip joint was possible to 90 degrees. The same procedure was...
Multiple stress fractures in a young female runner.
T Dusek, M Pećina, M Loncar-Dusek, I Bojanic
Acta Chir Orthop Traumatol Cech. 2004; 71(5):308-310
The effect of exercise on female's bone metabolism has received much attention in recent years. We report on unusual case of a female runner with low body mass and amenorrhea, who suffered 4 stress fractures. Three of the stress fractures occurred during her sports career, and the fourth occurred 7 years after the cessation of sports activities. It seems that exercise-induced amenorrhea together with food restriction in the young age may cause long-term consequences on bone metabolism.
[Non-union in os scaphoideum and its surgical treatment: overview of literature].
P Manák, P Drác
Acta Chir Orthop Traumatol Cech. 2004; 71(5):311-315
Authors present an overview of surgical methods used for the treatment of os scaphoideum with a focus on currently most frequent operations. In the period between January 1998 and March 2004, the authors performed 100 operations on the scaphoid bone, of this 54times primary fixation and 46times surgery for non-union in 42 patients. The most frequent method was resection of non-union, implantation of tri-cortical bone graft and internal fixation (17times). This method is considered by the authors optimal in non-unions in the middle third of the bone without signs of avascular necrosis of the roximal pole. In 8 patients with signs of avascular necrosis...
