With an ever increasing percentage of elderly people among the population fracture management in the aged is of increasing importance in the field of traumatology, since people between the ages of 60 and 80 will constitute the largest subpopulation by 2030. Due to relevant interindividual differences concerning physical constitution and comorbidity this group resembles an extremely inhomogenic population of patients. Discrepancies between biologic and chronologic age make a careful selection of suitable therapeutic strategies individually adapted to a single patient's situation necessary. For the biologically younger patient with good compliance conservative...

P Valis, R Chaloupka, M Krbec, M Repko, J Adler, M Nýdrle
Acta Chir Orthop Traumatol Cech. 2004; 71(6):339-344
PURPOSE OF THE STUDY: To present the results of a new method for treatment of cartilage defects of the patella, using a solid graft produced by chondrocytes on a three-dimensional matrix.MATERIAL AND METHODS: Chondrocyte transplantation is indicated in deep and extensive chondral and osteochondral defects of all large joints in patients at 18 to 40 years of age. When, on arthroscopic examination, a defect of patellar cartilage was found, healthy cartilage was collected from a non-weight-bearing surface, i. e., the intercondyllar fossa. In the tissue bank, the cartilage was fragmented, chondrocytes were isolated enzymatically and cultivated in vitro...

Acta Chir Orthop Traumatol Cech. 2004; 71(6):345-351
PURPOSE OF THE STUDY: To evaluate the efficacy and usefulness of 99Tc-MDP bone scans for the diagnosis of complications associated with hydroxyapatite- coated total hip prostheses.MATERIAL AND METHODS: 99Tc-MDP bone scans of 42 hips were taken before revision surgery in 41 patients with ABG 1 prostheses. Of these, 26 patients (27 hips) were included in our study on the basis of the following criteria: at least 23 months between the index surgery and scintigraphic examination and the availability of good quality bone scans. The average patients' age at the time of index and revision operations was 45.3 years (range, 32-57; SD, 6.02) and 50.2 years...
[Total knee arthroplasty with the Beznoska S.V.L. implant: short-term results].

Acta Chir Orthop Traumatol Cech. 2004; 71(6):352-359
PURPOSE OF THE STUDY: To present and evaluate short-term clinical results of total knee arthroplasty using the Beznoska S. V. L. implant and to compare them with the results of other authors.MATERIAL: In the period from April 1997 to October 2002, 197 total knee replacements with a S. V. L. implant were carried out of these, 164 implants were evaluated in 146 patients. This group comprised 116 women (78 %) and 48 men (22 %), with an average age of 73.6 years (range, 63 to 83) at the time of surgery. The patients were evaluated on average at 4.3 years (range, 1.5 to 6.5) after surgery. Indications for replacement were primary knee arthritis in 151,...
[Bone surgery for unstable hips in patients with cerebral palsy].
J Poul, M Pesl, M Pokorná
Acta Chir Orthop Traumatol Cech. 2004; 71(6):360-365
PURPOSE OF THE STUDY: The aim of this retrospective study was to compare the efficacy of femoral osteotomy alone with that of osteotomy combined with an acetabular procedure in patients with unstable hips due to spastic cerebral palsy.MATERIAL: Sixty-one hip joints in 50 patients who had shown distinct subluxation or dislocation of the joint were operated on. Eleven patients underwent bilateral surgery. Before bone surgery, soft-tissue release involving both the flexors and adductors was performed on 19 hips. Femoral osteotomy alone was performed on 29 hip joints and combined femoral and pelvic osteotomy was carried out on 32 joints.Twelve resections...

Acta Chir Orthop Traumatol Cech. 2004; 71(6):366-372
PURPOSE OF THE STUDY: The authors present a retrospective evaluation of a group of patients with injury to the lower cervical spine. The aim of the study is to show current trends in the diagnosis and treatment of this trauma.MATERIAL: Between 1995 and 2003, 363 patients with trauma to the lower cervical spine were treated in our clinic. They included 63 women and 300 men, who fell into age categories as follows:14-19 years, 52 patients; 20-29, 108 patients; 30-39, 76 patients; 40-49, 57 patients; 50-59, 40 patients; 60-69, 22 patients; over 70, 8 patients. Neurological findings were classified as Frankel A in 109 patients, Frankel B in 24 patients,...
[Options for correction of post-traumatic length discrepancies in forearm bones in children (case reports)].

Acta Chir Orthop Traumatol Cech. 2004; 71(6):373-378
Correction of radius length after a premature complete or partial closure of the distal epiphyseal plate is important for both the correction of radial deviation of the hand and the restoration of normal function of the radiocarpal articulation. This study comprised four patients. Two children with complete closure of the epiphyseal plate of the distal radius underwent callotaxis. One patient was treated for atrophic pseudoarthrosis of the distal radial metaphysis by external fixation and subsequent spongioplasty. One patient with partial closure of the epiphyseal plate was treated by physeal distraction. In each patient an Ilizarov's external fixator...
