Acta Chir Orthop Traumatol Cech. 2007; 74(1):5-13 | DOI: 10.55095/achot2007/001
PURPOSE OF THE STUDYAnatomic changes associated with aseptic loosening make conditions for revision of total knee arthroplasty more difficult. The aim of this study was to evaluate the results of revision total knee replacement at an average follow-up of 6.1 years.MATERIAL AND METHODSA total of 97 revision knee replacements due to aseptic loosening carried out in the years 1992 to 2003 were evaluated. The group included 46 men and 51 women at an average age of 66.8 years. The average preoperative Knee Society Score (KKS) was 31 points and the Functional Score (FS) was 22 points. There were 41 minor operations for AORI type I defects, 49...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):14-18 | DOI: 10.55095/achot2007/002
PURPOSE OF THE STUDYReinforcement rings are widely used for treatment of large acetabular defects but significant migration due to a lack of implant integration into the bone is a common problem. Although insufficient congruence between implant and acetabular bone has been identified main factor in early implant loosening, there are no reconstruction ring design parameters based upon anatomical data of the pelvis available.MATERIALIn this study such parameters were calculated on the basis of standardized computer tomograms of the hip region of 10 male and 10 female patients (40 data sets).METHODSThe center of rotation, the acetabular...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):19-28 | DOI: 10.55095/achot2007/003
PURPOSE OF THE STUDYThe authors present a group of patients treated for pelvic fractures in a period of 6 years and they evaluate radiographic findings and clinical outcomes following surgical management of type B and type C fractures.MATERIAL AND METHODSBetween July 1998 and June 2004, a total of 271 patients with pelvic fractures, 162 men and 109 women (average age, 42 years; range, 15 to 93 years) were hospitalized at the authors' departments. Of these, 141 patients were operated on (94 men, 47 women; average age, 37 years; range, 15 to 72 years) and 130 were treated conservatively (average age, 47 years; range, 15 to 93 years).The...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):29-36 | DOI: 10.55095/achot2007/004
PURPOSE OF THE STUDYThe authors present the results of surgical treatment of talar dome cartilage defects. They used the transplantation of autologous cultured chondrocytes in the form of a solid chondral graft.MATERIAL AND METHODSPatients with chondral lesions categorized as grades II to IV by the Anderson classification or as grades II to IV by the Berndt and Harty classification were indicated for this treatment. After preoperative MRI examination, a small sample of healthy cartilage was harvested arthroscopically from the non-weight-bearing area of the talus and was sent to the Tissue Bank in Brno for chondrocyte cultivation. After...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):37-46 | DOI: 10.55095/achot2007/005
PURPOSE OF THE STUDYThe management of intracapsular femoral neck fractures remains an unsolved issue of the present-day trauma surgery of the musculoskeletal system. These fractures are conventionally treated with the use of spongious screws or a dynamic hip screw combined with an antirotational spongious screw. The aim of this study was to evaluate the outcomes of surgical therapy using a different type of implant (proximal femoral nail) for intracapsular femoral neck fractures.MATERIALFrom January 2000 to March 2004, intracapsular fractures were treated with the use of a proximal femoral nail in 70 patients. In this retrospective study,...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):47-54 | DOI: 10.55095/achot2007/006
PURPOSE OF THE STUDYThe aim of the study was a retrospective evaluation of our patients with post-dysplastic hips treated by cemented or non-cemented total hip arthroplasty (THA) in order to ascertain which type and position of the acetabular component was most effective.MATERIALIn the years 1999-2002, 111 THA procedures were performed in 93 patients, 76 women and 17 men, with post-dysplastic hip arthritis. The average age of the patients at the time of implantation was 52.6 years. On the basis of pre-operative radiographic findings, the patients' conditions were evaluated using the Hartofilakidis classification into three disease categories:...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):55-58 | DOI: 10.55095/achot2007/007
PURPOSE OF THE STUDYScapholunate (SL) ligament disruption frequently occurs together with distal radius fractures. If it is not diagnosed and treated, instability develops and will eventually be manifested as arthritic lesions known as scapholunate advanced collapse (SLAC) of the wrist. The aim of this study was to record the occurrence of SL ligament disruption in the patients treated conservatively for displaced fractures of the distal radius and to find out which fracture types are most often associated with injury to the SL ligament.MATERIAL AND METHODSA total of 75 patients treated conservatively for distal radius fractures in 1997-98...

Acta Chir Orthop Traumatol Cech. 2007; 74(1):59-64 | DOI: 10.55095/achot2007/008
PURPOSE OF THE STUDYThe CF 30 stem in combination with a cementless acetabulum was used at the First Department of Orthopedic Surgery in Brno in the years 1994 to 1995. From the second year following implantation, aseptic stem loosening was recorded. In order to find explanation of this early loosening, the authors, in cooperation with the Institute of Solid Mechanics, Mechatronics and Biomechanics, carried out the stress-strain analysis in a model system.MATERIALEighty patients (31 men and 49 women) received a cemented CF30 femoral component in 1994. Of them, 16 patients underwent revision arthroplasty, three died of causes unrelated to...
