Acta Chir Orthop Traumatol Cech. 2007; 74(2):79-90 | DOI: 10.55095/achot2007/011
According to the available sources, no case of total spondylectomy of C2 with preservation of roots, preservation of vertebral arteries and a short fixation without occipitocervical fusion has been so far described in the literature. We decided to perform a radical surgery in a man, now 27 y. o., with solitary metastasis of thyroid adenocarcinoma. In the first step, we applied the posterior surgical approach. The patient was placed prone on a standard operating table with a support of head fixed by adhesive plaster, with the upper cervical spine slightly bent forward. We made a mid-line incision, extending from the external occipital protuberance...

Acta Chir Orthop Traumatol Cech. 2007; 74(2):91-98 | DOI: 10.55095/achot2007/012
PURPOSE OF THE STUDYA group of 79 patients with ipsilateral fractures of the hip and femoral shaft treated with the use of a long proximal femoral nail (PFN-long) was retrospectively evaluated.MATERIALFrom January 1998 to February 2005, 79 patients were treated surgically. The group included 42 men and 37 women at an average age of 56.6 years. In 47 patients (37 men and 10 women; average age, 38 years; range, 18-72 years) the fractures were due to a high-energy trauma. In the remaining 32 patients (six men and 25 women; average age, 77.6 years), the cause of fracture was a low-energy trauma, most frequently an ordinary fall. The fractures...

Acta Chir Orthop Traumatol Cech. 2007; 74(2):99-104 | DOI: 10.55095/achot2007/013
PURPOSE OF THE STUDYThe femoral head of a bipolar hip prosthesis (BHP) consists of a steel outer shell and polyethylene liner with an inner steel head moving inside. In this study, indications for and advantages of this endoprosthesis are evaluated.MATERIAL AND METHODSThe author presents indications and the surgical technique on a group of 79 patients, mostly physically active women aged 70 to 80 years. A BHP is largely indicated in patients with femoral neck fractures, and then in injuries to the proximal femur that leave the articular surface of the acetabulum intact.RESULTSIn the group of 79 patients, four had dislocation of...

Acta Chir Orthop Traumatol Cech. 2007; 74(2):105-110 | DOI: 10.55095/achot2007/014
PURPOSE OF THE STUDYHallux valgus is a frequent static deformity of feet in shoe-wearing populations. Lasting problems usually require surgical management. The authors evaluate the long-term results of such treatment by either McBride's operation or chevron osteotomy, or by combination of both.MATERIALA group of 72 patients with hallux valgus underwent 84 operations, with the use of McBride's procedure, chevron osteotomy or a combination of both, at the First Department of Orthopedic Surgery, St. Anne's Teaching Hospital in Brno, in the years 1993-1995. At 10-year follow-up they were evaluated on the basis of patients' subjective satisfaction...

Acta Chir Orthop Traumatol Cech. 2007; 74(2):111-113 | DOI: 10.55095/achot2007/015
PURPOSE OF THE STUDYOrthopedic tuberculosis is number one in non-pulmonary forms of tuberculosis and, among these, knee joint tuberculosis ranks second after spinal tuberculosis. Arthritic lesions are currently treated by total knee arthroplasty (TKA).MATERIALA group of 10 patients who, after having gone through tuberculous gonarthritis, underwent TKA was evaluated for the period from 1980 to 2005. It consisted of six women and four men, whose average age at the time of arthroplasty was 65 years.METHODSThe most frequent indication for TKA after tuberculous gonarthritis was post-inflammatory arthritis (80 %); the other indication...

Accurate Placement of Bone Tunnels in Reconstruction of the Anterior Cruciate Ligament - a Contribution of Computer-Assisted NavigationCurrent concepts review

Acta Chir Orthop Traumatol Cech. 2007; 74(2):118-125 | DOI: 10.55095/achot2007/017
Rupture of the anterior cruciate ligament (ACL) is one of the most frequent injuries to the knee joint in the young. ACL repair is a major orthopedic procedure most often performed in the younger adult population. Early stabilization of the knee joint by ACL reconstruction also decreases the risk of injury to other important structures. At ACL reconstruction, the biggest problem is usually the exact placement of drilled tunnels. This significantly affects the outcome of surgery, i. e., range of motion, knee joint stability, reaction of the synovium in the knee, pain, impingement and potential graft failure with lesion development. However, 70 % of...

Acta Chir Orthop Traumatol Cech. 2007; 74(2):114-117 | DOI: 10.55095/achot2007/016
A fifty-year-old woman with developmental dysplasia of the hip underwent total hip arhtroplasty, and subsequently developed recurrent venous thrombophilia of the lower extremities. Hematological examination revealed an inherited disorder of blood coagulation (homozygous mutation of the 5,10-methylenetetrahydrofolate reductase gene) and therefore long-term Warfarin anticoagulation therapy was started. A year later she was diagnosed with a large pelvic posthemorrhagic pseudocyst (hematoma) located below the musculus iliacus and adhering to bone in the region of posterior acetabulum. The condition was complicated by usuration and focal osteolysis of the...
