Acta Chir Orthop Traumatol Cech. 2007; 74(3):162-170 | DOI: 10.55095/achot2007/026
PURPOSE OF THE STUDYThe aim of the study was to evaluate the results of treatment in patients with infected fractures or chronic osteomyelitis in the tibial and ankle regions by radical debridement of skeletal and soft tissues and coverage of the defect, using free flap transfer or muscle flap transposition.MATERIALBetween January 1, 2002 and December 31, 2004, 26 patients were treated by this method; 13 had infected tibial or ankle fractures and 13 showed osteomyelitis of the tibia. All patients, in the case of failure, would have been indicated for below-knee amputation. The group comprised six women and 20 men at an average age of 46...
On the Outcome of Computer-assisted Total Knee ReplacementOriginal papers

Acta Chir Orthop Traumatol Cech. 2007; 74(3):171-174 | DOI: 10.55095/achot2007/027
A number of studies have reported a significant improvement of the postoperative alignment, when computer-navigated total knee arthroplasty (TKA) was compared with conventional techniques. However, no studies are available on the functional and patient-relevant outcomes after computer-assisted knee replacement. In a prospective, randomized trial comparing 27 computer-assisted TKAs with 25 conventional implantations, the Knee Society Score was used to assess functional status, and the WOMAC questionnaire was used to record the disease-specific, patient-relevant outcome. At a twelve-month follow-up no significant difference was detected between the two...
Ten-Year Experience with the Aldinger Custom-made Total Hip Arthroplasty: Evaluation of the ResultsOriginal papers
J. MASÁR
Acta Chir Orthop Traumatol Cech. 2007; 74(3):175-181 | DOI: 10.55095/achot2007/028
PURPOSE OF THE STUDYThe author presents the results of treatment with total hip arthroplasty (THA), in the Aldinger modification, in 91 patients receiving a total of 98 custom-made total hip implants in the period from 1993 to 2002. Eighty-eight patients (96.9 %) with 95 THAs were available for evaluation at the last follow-up.MATERIALThe group comprised 26 men (27 total hip implants), at an average age of 33.8 years (range, 25-57), and 65 women (71 implants) at an average age of 38.5 years (23-57) at the time of surgery. The right hip was operated on in 43 patients (18 men and 25 women) and the left one in 55 patients (9 men and 46 women)....

Acta Chir Orthop Traumatol Cech. 2007; 74(3):182-188 | DOI: 10.55095/achot2007/029
PURPOSE OF THE STUDYIn this double-blind prospective study, pain after reconstruction of the anterior cruciate ligament (ACL) was evaluated using the visual analogue scale (VAS). Comparisons were made between patients administered an intra-articular analgesic mixture of adrenaline, morphine and bupivacaine (Marcaine) and those without it, between patients surgically treated by the BTB technique and those undergoing hamstring tendon ACL reconstruction, and between men and women undergoing the same procedure.MATERIALEighty-five randomly selected patients were allocated by five groups according to the surgery performed: 1. ACL reconstruction...

Acta Chir Orthop Traumatol Cech. 2007; 74(3):189-194 | DOI: 10.55095/achot2007/030
PURPOSE OF THE STUDYCervical spine injuries in young adults are usually caused by high-energy trauma. However, a typical injury to the cervical spine can also occur in older patients, in whom it is often associated with the presence of osteoporosis and relatively low-energy trauma, similarly to distal radial or proximal femoral fractures, or fractures of the thoracolumbar spine. The aim of this study was to evaluate a group of elderly patients with cervical spine injuries treated at our department.MATERIALIn the period from 2001 to 2005, 66 patients older than 65 years were treated for cervical spine injury at the Department of Spinal Surgery...

Acta Chir Orthop Traumatol Cech. 2007; 74(3):195-201 | DOI: 10.55095/achot2007/031
PURPOSE OF THE STUDYThe authors evaluated the mid-term results in 133 patients to whom 143 uncemented Beznoska (BMT) acetabular cups were implanted.MATERIALA total of 143 acetabular cups were implanted to 133 patients in the period from 1994 to 2002. Thirteen patients died or dropped out of the study during follow-up.METHODSThe patients were followed up at 3, 6 and 12 months after surgery and then, according to their conditions, every one or two years. In each patient, the following information was recorded: age at the time of implantation, gender, pre-operative diagnosis, physical activity, body mass index, Harris hip score, acetabular...

Members of the medical profession spend a long time and financial resources acquiring the knowledge necessary for the discharge of their responsibilities; therefore, they should be appropriately compensated for the services they provide. However, it has become obvious, at least in the United States, that excessive emphasis on profit making is fostering the transformation of the medical profession into strictly a business. This transformation is spawning ethical infractions, which are becoming increasingly apparent.

Patients suffering form epilepsy have an increased risk for fractures. Beside fractures caused by fall or accident muscles forces alone generated during tonic-clonic seizure can result in severe musculoskeletal injury. Contractions of strong paraspinal muscles can lead to compression fracture of the mid-thoracic spine. We report a patient who had suffered from a tonic-clonic seizure during early morning hours. After a cracking sound the patient woke up in a state of post-ictal disorientation, loss of urine and tongue bite. He was admitted to our facilities with the suspected vertebral fracture albeit he just reported of mild lower back pain. Native...

Acta Chir Orthop Traumatol Cech. 2007; 74(3):206-209 | DOI: 10.55095/achot2007/033
Tuberculous hip arthritis ranks third in the classification of orthopedic tuberculosis, after tuberculosis of the spine and knee joint. It accounts for about 15 % of all orthopedic tuberculosis. The aim of this paper is to demonstrate the etiology, clinical presentation and forms of disease on the case reports of six patients. The scope of disease is wide - from a mild reversible synovial form, then fibrous or bone ankylosis, to severe destruction of the hip with subluxation or dislocation, pseudoarthrosis or chronic arthritis with a fistula. The wide diversity of clinical findings and difficulties is making diagnosis are often responsible for a...

Currently, conventional diagnosis of acetabular fractures is often supplemented and even replaced by CT examination, as this can provide a more detailed image of the extent of fracture and its morphology. Supported by developmental anatomy, Harris et al. proposed a new classification for acetabular fractures in which the pubic bone component of the acetabulum is shown to coincide with the anterior column of the acetabulum. There is no need to abandon the generally accepted and much used Letournel-Judet classification, but changes in the algorithm of acetabular fracture examination will certainly require that the CT classification of acetabular fractures...
