BACKGROUNDIn this retrospective study we describe our experiences in minimally invasive two incision total hip replacement (THR).METHODSWe performed 150 fluoroscopy-assisted two incision minimally invasive THR on 148 patients using a proximally coated hydroxyapatite implant. Clinical (Harris Hip Score (HHS), surgical time, blood loss, length of hospital stay) and radiographic data (leg length discrepancy, cup abduction angle, stem alignment) were collected.RESULTSThe HHS increased from 54.7 preoperatively to 92.6 postoperatively. Mean surgical time was 103 minutes. Mean intraoperative blood loss was 410 ml. There were 11 (7.3%)...

PURPOSE OF THE STUDYThe aim of the study was to evaluate the results of primary total hip arthroplasty with the use of the CLS stem at 11 to 17 years after implantation.MATERIAL AND METHODSA total of 108 patients (122 hips) in whom a CLS stem was used in the 1991-1996 period were evaluated. The group included 34 men and 74 women, with an average age of 48 years (range, 28-63). The CLS stem with a neck-shaft angle of 145° and the CLS expansion cup were used in all patients. Clinical outcomes were evaluated by Merle d'Aubigné-Postel score and Harris hip score, radiological examination was completed on AP and lateral views of the pelvis and...

PURPOSE OF THE STUDYMinimally invasive surgery (MIS) techniques have recently become a powerful and effective marketing instruments that are often perceived by the patient as the criterion of the surgeon's and institution's standard. In addition to studies reporting the benefits of minimally invasive procedures, some authors have recently found no such benefits or even pointed out some disadvantages. In this paper we present our own view of this issue. Our definition of minimally invasive surgery: a minimally invasive procedure is such that an optimally placed incision using anatomical intervals without damage to muscle insertions allows us...

PURPOSE OF THE STUDYDevelopmental dysplasia of the hip (DDH), treated either conservatively or surgically, may result in a dysplastic acetabulum. To treat this after bone maturation is completed, Bernese periacetabular osteotomy, also known as the Ganz procedure, has recently been used as the method of choice at our department. The evaluation of the first results is presented here.MATERIAL AND METHODSThe group evaluated comprised 25 hips (21 female and 4 male) in 22 patients. Nineteen hips were treated for residual dysplasia of the acetabulum due to DDH, and six for a spastic hip. The average age was 26 years (range, 12 to 44) and 14 right...

PURPOSE OF THE STUDYProsthetic joint infection (PJI) is a feared complication of total joint arthroplasty. Several strategies have been developed to treat it. The purpose of our study was to compare therapeutic strategies for PJI treatment published in the literature.MATERIAL AND METHODSWe retrieved around 5,000 documents concerning this topic, published between January 1960 and November 2006, from the databases MEDLINE and EMBASE. Using a two-phase selection, 382 relevant articles were chosen, and full texts were obtained for 302 of them (79%). However, only 77 of them fulfilled the criteria for inclusion in our study. A total of 645 hips...

Acta Chir Orthop Traumatol Cech. 2009; 76(4):310-313 | DOI: 10.55095/achot2009/056
PURPOSE OF THE STUDYDamage control surgery is one of the major advances in surgical techniques used in polytrauma patients in the past 25 years. It is based on a foreshortened pre-operative examination during ongoing resuscitation which includes surgical intervention in an injured patient with severe hypotension due to haemorrhage. In this seven-year retrospective study the authors evaluate the results of damage control laparotomy in patients with injury to the abdominal organs, retroperitoneum and pelvis.MATERIAL AND METHODSThe evaluated group included 21 patients with blunt abdominal trauma who underwent damage control laparotomy in the...

PURPOSE OF THE STUDYOne of the methods used for treatment of Kienböck's disease is based on transposition of the pisiform bone into free space created by removal of the lunate bone. It is performed in patients with stage IIIB to IV, as assessed by Lichtmann's score. However, this operative procedure has so far lacked an unequivocal assessment of its therapeutic value. The aim of our work was to assess the therapeutic effect of the Kuhlmann method in the treatment of advanced stages of Kienböck's disease.MATERIALFrom January 1996, eighteen patients (18 wrists) diagnosed with Kienböck's disease were operated on, using the Kuhlmann method,...

PURPOSE OF THE STUDYThe treatment of femoral neck fractures shows a relatively high number of poor outcomes, usually due to late complications, such as avascular necrosis of the femoral head or pseudoarthrosis. The latter may develop when the osteosynthesis of osteoporotic bone fails. The aim of this retrospective study was to evaluate a group of patients treated by osteosynthesis for intra-capsular femoral neck fractures at our department, and to verify indication criteria and identify the therapeutic procedures that are best suited to our conditions.MATERIALIn the 1997-2001 period, a total of 81 patients with intra-capsular femoral neck...

PURPOSE OF THE STUDYTo present the results of surgical treatment of carpometacarpal joint arthritis of the thumb, using cannulated screw fixation which is stable and, after bony union, allows the patient to do also harder manual work.MATERIALIn the years 2000 to 2005, this method was used for 20 operations in 18 patients (16 women and two men); one man and one woman had bilateral surgery. The right hand was operated on in seven cases and the left one in 13 cases. The average age of the patients was 61 years.METHODSThe procedure was performed from the dorsoradial approach, access to the joint was gained between the extensor pollicis...

Traumatic obturator dislocation of the hip without fracture associated with antero-lateral knee dislocation is considered to be an extreme injury. Such a case, which has never been treated in our department, is reported here. The patient was a 40-year-old man who suffered his injury in a motorcycle accident. He had ipsilateral traumatic obturator hip dislocation and anterior knee dislocation. Within two hours of injury dislocation reduction was carried out under general anaesthesia, the anterior cruciate ligament was reconstructed using BTB allograft and a CT scan was obtained. In this type of trauma CT-angiography is always indicated to detect any...

Acta Chir Orthop Traumatol Cech. 2009; 76(4):335-337 | DOI: 10.55095/achot2009/061
The authors describe the case of a patient, a 33-year-old man, who presented with paresis of the hypoglossal nerve following an occipital condyle fracture. The patient, as a driver, was involved in a car accident and from the beginning he complained of neck pain. The diagnosis was established by CT examination as late as 72 hours after injury on the basis of clinical presentation (difficulty in swallowing, chewing and speaking due to impaired mobility of the left half of the tongue, with the left half showing a marked hypotrophy and muscle weakness). A conservative treatment was used - application of a rigid cervical collar for 3 months, which resulted...

The aim of the paper is to provide information on the establishment of the Geriatric Fracture Centre (GFC) at the Department of Orthopaedics and Traumatology, Faculty Hospital at the Faculty of Medicine, Charles University, Pilsner, whose goals and services are in accordance with the principles of the Rochester GFC, USA, and the AO Foundation at Synthes Inc. The paper presents up-to-date information on the methods of osteosynthesis for the skeletal system affected by osteoporosis, the majority of which was obtained in the AO Geriatric Fracture Course in Davos, December 2007. The authors are concerned with the pre-operative examination of geriatric...
