PURPOSE OF THE STUDYThis retrospective study reports on the clinical results of a group of 23 patients with subtrochanteric femoral fractures using the Long Trochanteric Fixation Nail (LTFN).MATERIALBetween January 2005 and January 2008, 23 patients (20 women, 3 men; average age: 64.8 years old) with subtrochanteric femoral fractures were treated surgically. According to the AO/ASIF Classification, the most frequent fracture type was an 32-A1. They were also classified regarding the Seinsheimer Classification, in which the commonest type was the IIB. Of the 23 fractures, 14 of them had been the result of an unexpected fall, 2 were the result...

Acta Chir Orthop Traumatol Cech. 2009; 76(6):456-461 | DOI: 10.55095/achot2009/085

PURPOSE OF THE STUDYPrevious surface EMG studies have shown that chronic rotator cuff tears (RCT) may be associated with a altered activation of adjacent shoulder muscles. The effect of RCT on central neuromuscular control mechanisms of the shoulder girdle muscles such as the deltoideus muscle (MD), a key muscle of shoulder function, has as yet not been studied in detail. This study investigated the cortico-spinal excitability of the MD to assess the effects of RCT on the central neuromuscular function of upper limb muscles.MATERIAL AND METHODSThe motor evoked potentials (MEP) in response to transcranial magnetic stimulation of MD and first...

PURPOSE OF THE STUDYA prerequisite for the successful implantation of an acetabular component in a defective bone tissue is, in addition to providing sufficient primary fixation of the implant, reconstruction of the acetabular bone bed. To achieve this, one of the options involves the use of an oblong implant. Data published on this topic are sparse and therefore the aim of this study was to gain a deeper insight into this issue. In addition, we wanted to compare the results achieved with the oblong component with those obtained using its original model.MATERIALOne group comprised 111 patients with 133 sides who underwent acetabular revision...

Acta Chir Orthop Traumatol Cech. 2009; 76(6):473-478 | DOI: 10.55095/achot2009/087

PURPOSE OF THE STUDYThe frequency of periprosthetic fractures related to total knee arthroplasty is increasing, with a prevalence of 1.3 % on the average and with women being affected more often (4 out of 5 patients). Fractures of the distal femur are common, while tibial fractures are rare. Crucial for treatment is to distinguish fractures of the metaphysis above the femoral component, which remains firmly fixed, from those involving the knee joint replacement and component loosening. Supracondylar periprosthetic fractures are almost always managed surgically, using methods of osteosynthesis with an angle condylar or DCS plate, or a short retrograde-inserted...

Acta Chir Orthop Traumatol Cech. 2009; 76(6):479-486 | DOI: 10.55095/achot2009/088

PURPOSE OF THE STUDYOccipitocervical fixation and spondylodesis is indicated in various cases of occipitocervical instability. The aim of this retrospective study was to evaluate the results of occipitocervical fixation at our institutions.MATERIALBetween 1997 and 2007, a total of 57 patients underwent occipitocervical fixation (OC) there were 25 men and 32 women, from four to 77 years of age, with an average of 58.7 years. The patients were allocated to two groups according to the method of OC fixation used: tying wires or cables (group 1) screw-rod or screw-plate systems (group 2). Indications for OC fixation included trauma in 15, rheumatoid...

PURPOSE OF THE STUDYThe authors evaluate the mid-term results and their experience with the cementless total hip replacement Beznoska type S.F. "Slavík, Fencl" in a group of 40 patients.MATERIALDuring 2002, a total of 46 patients underwent implantation of a cementless total hip replacement (THR) type S.F. Six patients failed to come to the final follow-up at 5 years after surgery. Coxarthritis was the indication for primary surgery in the majority of the patients. The average age of the group, which comprised 20 men and 20 women, was 63 (50-75) years.METHODSAt the end of 2007, 40 patients were evaluated at an average follow-up of...

Acta Chir Orthop Traumatol Cech. 2009; 76(6):495-500 | DOI: 10.55095/achot2009/090

PURPOSE OF THE STUDYOwing to advances in operative techniques and biotechnology, bone replacement biocompatible materials have recently come into focus for orthopaedic and trauma surgeons. Bone lytic lesions, such as tumorous bone defects, diseases simulating cancer, chronic inflammatory lesions or skeletal injuries, often require stabilisation of the skeleton and treatment of the bone affected. Juvenile bone cysts are benign lytic lesions posing a threat to bone compactness in childhood. They are benign, fluid containing bone cavities, lined with a membrane consisting of thin vascularised connective tissue with scattered osteoclast-like cells....

Acta Chir Orthop Traumatol Cech. 2009; 76(6):501-504 | DOI: 10.55095/achot2009/091

PURPOSE OF THE STUDYWe present a group of patients with spinal metastatic disease surgically treated at our department, with an evaluation of their neurological outcomes in relation to the initial disease and the surgical technique used.MATERIALBetween 1989 and 2007 we operated on 748 patients with spinal tumour. Of these, 380 had metastatic disease. The Frankel classification system was used to assess neurological status.METHODSBased on the Tomita prognostic score, the metastatic disease was evaluated and appropriate surgical procedure was selected (biopsy, posterolateral decompression, posterolateral decompression with stabilisation,...

In this case study, three patients are presented who had incomplete cauda equina syndrome following elective lumbar spine surgery for degenerative disease. In all patients, the neurological symptoms developed due to post-operative arachnoiditis. Its aetiology, pathogenesis and diagnostics are discussed, as well as the methods of prevention and therapy which are still limited and often not beyond experimentation.

Acta Chir Orthop Traumatol Cech. 2009; 76(6):509-512 | DOI: 10.55095/achot2009/093

Reimplantation of a total hip arthroplasty (THA) with excessive heterotopic polyethylene granuloma is reported. An 84-year-old woman, who had undergone THA reimplantation on the right side 4.5 years previously, was indicated for revision surgery due to aseptic failure of the acetabular component. Pre-operative examination revealed a pulsating mass below the right inguinal ligament that raised the suspicion of a pseudoaneurysm of the external iliac artery. Because of this suspected late complication of the first revision arthroplasty, surgical exploration was indicated by the vascular surgeon prior the second reimplantation. A CT-scan showed a large...
