INTRODUCTIONInformation on the distribution of UHMWPE wear particles around joint replacements is essential for the understanding of aseptic loosening of implants.MATERIAL AND METHODSSamples of soft tissue from around total hip arthroplasty (THA), which had a typical appearance of polyethylene granuloma, were collected from the places corresponding to the radiological zones of Gruen and DeLee. The samples were taken from 45 patients (group N1). Since the samples varied in macroscopic appearance and/or the technique of processing, a sub-group of 15 more uniform samples (group N2) were collected by the first author in order to ensure...

PURPOSE OF THE STUDYThe Stoppa approach used in acetabular and pelvic ring fractures provides an excellent visualisation of the anterior column, quadrilateral plate and part of the posterior column for its exact reduction. Our first experience of this surgical approach and preliminary results are reported.MATERIAL AND METHODSBetween April 2008 and September 2009, the Stoppa approach was used 15 times in 14 patients. This series includes 13 males and one female with an average age of 47.7 years. The mean follow-up was 6.8 months (range from 3 to 15) in 11 patients. In three patients, the post-operative period was too short for evaluation....

PURPOSE OF THE STUDYTo evaluate the effect of complications on the quality of life in patients after elective stabilisation surgery on the lumbar spine.MATERIAL AND METHODSBetween January 2005 and June 2007, 208 patients (120 women and 88 men) were included in the prospective study carried out at our department. These patients were undergoing elective surgery for lumbar spinal disease, namely, central and lateral stenosis, degenerative disc disease and degenerative and isthmic spondylolisthesis. All patients were treated by transpedicular fixation and fusion involving transforaminal lumbar interbody fusion (TLIF) in 165 patients, anterior...

Is Kaplan-Meier Statistics the Most Appropriate Tool for Survivorship Measurement of Outcomes in Orthopaedics?Original papers

K. LANGOVÁ, J. GALLO

Acta Chir Orthop Traumatol Cech. 2010; 77(2):118-123 | DOI: 10.55095/achot2010/022

PURPOSE OF THE STUDYAn assessment of the results over a time period is an integral part of any treatment evaluation. A standard method used for this purpose is Kaplan-Meier survival analysis. The aim of this study was to show how, for total hip arthroplasty (THA), estimates of prosthesis survival probability can be affected by a factor known as competing events.MATERIAL AND METHODSA set of input data concerning competing events (death and loss to follow-up), in addition to the investigated event (revision THA), was analysed by both the Kaplan-Meier method and the method of cumulative incidence with the use of worst-case analysis.RESULTSOur...

Adjacent Segment Degeneration after Lumbosacral Fusion in Spondylolisthesis: A Retrospective Radiological and Clinical AnalysisOriginal papers

P. ®ENČICA, R. CHALOUPKA, J. HLADÍKOVÁ, M. KRBEC

Acta Chir Orthop Traumatol Cech. 2010; 77(2):124-130 | DOI: 10.55095/achot2010/023

PURPOSE OF THE STUDYWhereas the posterior lumbar interbody fusion (PLIF) technique with pedicle screw fixation has shown satisfactory clinical results, solid fusion has been reported to accelerate degenerative changes at adjacent unfused levels, especially at the cranial level. The aim of this retrospective study was to evaluate a group of patients with adjacent segment disease (ASD) developed after 360-degrees lumbar fusion for spondylolisthesis performed by PLIF with transpedicular fixation and posterolateral fusion (PLF). Radiographic examinations were focused on the origin or progression of degenerative changes at the adjacent segments after...

Arthritis Development Following Untreated Scapholunate Instability of the WristOriginal papers

Acta Chir Orthop Traumatol Cech. 2010; 77(2):131-133 | DOI: 10.55095/achot2010/024

PURPOSE OF THE STUDYComplete disruption of the scapholunate (SL) ligament results in the development of arthritic lesions known as scapholunate advanced collapse (SLAC) of the wrist.MATERIAL AND METHODSThe authors present the results of a prospective study of seven patients in whom radiography showed an acute complete rupture of the scapholunate ligament and who refused SL ligament reconstruction. This group (1) was compared with a group of 17 patients (2) who underwent ligament reconstruction soon? after an acute rupture had been detected. All patients were examined at 36 months after injury for hand function and pain using the Wrightington...

Screw Loosening after Posterior Dynamic Stabilization - Review of the LiteratureOriginal papers

T. KOCAK, B. CAKIR, H. REICHEL, T. MATTES

Acta Chir Orthop Traumatol Cech. 2010; 77(2):134-139 | DOI: 10.55095/achot2010/025

PURPOSE OF THE STUDYThe malposition of pedicle screws in the lumbar spine is associated with a potential risk of iatrogenic injury of neurological structures but also with a decrease of biomechanical stability. The correct position of the pedicle screws of a dynamic stabilization device, as a long-term implant, seems to be of great importance. A high incidence of screw loosening could influence both the rate of revision surgeries and the clinical results. We compared screw loosening in our own patients with published data after navigated and non-navigated implantation of Dynesys.MATERIAL AND METHODSPosterior instrumentation with the Dynesys...

P. DRÁČ, P. MAŇÁK, I. ČI®MÁŘ, J. HRBEK, J. ZAPLETALOVÁ

Acta Chir Orthop Traumatol Cech. 2010; 77(2):143-148 | DOI: 10.55095/achot2010/027

PURPOSE OF THE STUDYNon-displaced or minimally displaced scaphoid waist fractures can be surgically treated using either the percutaneous volar or the limited dorsal approach. These techniques provide a possibility of early wrist physiotherapy throughout the healing period, better functional outcomes and a shorter sick leave duration. Many reports cite low complication rates. The purpose of this study is to compare both techniques based on the complication rate and functional outcomes.MATERIAL AND METHODSThe authors compared 42 patients treated for an acute scaphoid waist fracture by percutaneous osteosynthesis and 38 patients treated by...

Comprehensive Therapy of Symptomatic Vertebral HaemangiomasCase report

L. HRABÁLEK

Acta Chir Orthop Traumatol Cech. 2010; 77(2):149-153 | DOI: 10.55095/achot2010/028

Vertebral haemangiomas (VH) are usually asymptomatic and are often found incidentally at spinal examination by imaging methods. Nevertheless, some haemangiomas can be clinically manifested by various degrees of axial pain and neurological deficit. The aim of this report is to show that this is a complex issue that requires a comprehensive approach to its treatment. The author reports on three patients with aggressive forms of cervical and lumbar VH treated by radiation therapy or vertebroplasty and hemilaminectomy with resection of the intraspinal thoratic component of a tumour. He discusses his findings in relation to the scarce data found on this...
