PURPOSE OF THE STUDYAtlantoaxial osteoarthritis (AAOA) is a clinical syndrome with signs distinctly different from those of degenerative subaxial spine disease. Its diagnosis may long be delayed, partly because of insufficient knowledge and partly due to difficulties in interpreting both anteroposterior and lateral radiographs. The aim of this prospective study was to evaluate the first 27 AAOA patients treated at our department.MATERIALFrom 2001 we performed atlantoaxial fixation with fusion in a total of 29 patients with painful arthritis of the atlantoaxial complex. The 27 patients treated before the end of 2010 were enrolled in the...

PURPOSE OF THE STUDYMotion-preservation technologies for spinal disorders have evolved and come into use in the last decade. Three principal systems are currently available: total disc replacement, posterior neutralisation transpedicular system and interspinous implants. The aim of this retrospective study was to evaluate our group of lumbar total disc replacements at a follow-up of 2 years.MATERIAL AND METHODSA total of 42 disc prostheses were implanted in 37 patients. Of these, 31 with 35 artificial discs were followed up for 2 years. There were 11 men and 20 women with an average age of 42.9 years (range, 21 to 61 years). The indication...

PURPOSE OF THE STUDYThe aim of the study was to compare two options of how to get the correct anatomical position of both femoral tunnels, using the transtibial or the anteromedial portal technique, during anatomical double-bundle anterior cruciate ligament (ACL) reconstruction.MATERIAL AND METHODSA total of 36 patients, 29 men and seven women, underwent double-bundle ACL reconstruction between October 2009 and December 2010. Their average age was 26.5 years. The average interval between ACL injury and reconstruction was 7.4 months. A diagnostic arthroscopy for the treatment of cartilage and meniscal lesions was performed in 21 patients...

PURPOSE OF THE STUDYTo review the current methods of operative management of post-traumatic chronic radial head dislocation in chronic Monteggia lesions in children.MATERIAL AND METHODSPost-traumatic chronic dislocation of the proximal radius usually occurs following missed or ineffectively treated Monteggia fractures. The radial head is usually dislocated anteriorly. We performed open reduction of the radial head, reconstruction of the annular ligament and corrective osteotomy of the ulna in eleven patients. Three of our patients underwent ulnar lengthening via gradual distraction using an Ilizarov external fixator. This procedure...

PURPOSE OF THE STUDYTo identify some characteristics of bone repair capacity in elderly patients who undergo total hip arthroplasty, which requires good healing ability of bone for implant osteointegration and bone defect repair, particularly if revision arthroplasty is necessary.MATERIAL AND METHODSIn a group of 27 patients (mean age, 70±7 years; range, 60 to 81 years) a coincidence of osteoarthritis and osteopenia/osteoporosis was assessed, and mesenchymal stem cells (MSC) were isolated and their numbers, viability and proliferative capacity were evaluated. The MSC populations were examined for their behaviour on bone tissue scaffolds...

PURPOSE OF THE STUDYThe aim of the study was to evaluate the group of patients treated for varus arthritic knee by the method of high tibial osteotomy from the lateral approach and to assess its role in the present-day orthopaedic surgery which also offers other options such as distraction osteotomy from the medial approach, autologous chondrocyte transplantation or alloplasty.MATERIAL AND METHODSThe study included 101 patients, 41 women and 60 men, undergoing lateral valgus high tibial osteotomy at our department between 2003 and 2007. The age of patients at the time of surgery ranged from 34 to 61 years, with an average of 54 years. The...

PURPOSE OF THE STUDYThe patients with pelvic ring injury involve two groups: 1) young and middle-age persons, mostly men, with serious injury to the pelvic ring due to high-energy trauma; and 2) older patients, mostly women, with osteoporotic fractures due to a simple fall. The aim of this study was to show significant differences in the selected epidemiological characteristics between these two groups.MATERIAL AND METHODSThe group comprised 225 patients older than 15 years with pelvic fractures who were treated between January 1, 2007, and December 31, 2009.In this retrospective descriptive study, the patients' data on age, gender,...

This paper describes current treatment strategies of distal femoral fractures as well as their evidence based rationale. The treatment of distal femoral fractures has improved with the evolution of plating and nailing technologies. The commonly selected surgical approaches are outlined and surgical treatment techniques including both internal and external fixation are discussed.

Odontoid process fractures in patients with ankylosing spondylitis (AS) are rare and their finding together with subaxial cervical spine injury is a great exception. Neither the mechanism of such a combined cervical spine injury nor its surgical treatment has so far been reported in the relevant literature (MEDLINE). The authors present two such cases, one in a 30-and the other in a 74-year-old man. Both AS patients showed a common mechanism of injury sustained in a car accident, which involved hitting a solid barrier at 60 to 70 kilometres per hour, resulting in hyperextension of the cervical spine. In both patients the fractures were stabilised from...

Patients with rheumatoid arthritis (RA) often have foot problems. The subtalar and particularly talonavicular joints are affected most frequently. The posterior tibial tendon has an important role in mid-foot stability. In RA patients, chronic inflammation of this tendon or talonavicular joint arthritis can results in posterior tibial tendon rupture. This leads to a collapsed talonavicular joint and forefoot instability, first with talonavicular and later Chopart's joint involvement. This shows as a planovalgus foot, with the forefoot in pronation and the heel in valgus deviation. In a 61-year-old RA patient, ruptures of the posterior tibial tendon...

An ankle bone consisting of several fragments is a rare anatomical variant of the talus, in the literature described as talus partitus or frontal split. On radiographs or CT scans it presents as two or more relatively large fragments. The term talus partitus is sometimes incorrectly used for an accessory bone that occasionally develops behind the ankle bone and is called os trigonum.Talus partitus is an unusual developmental anomaly in which the talus is partitioned although it ossifies from a single centre. The cause of its split has not been explained although this may also be due to a previous unrecognized injury....
