Acta Chir Orthop Traumatol Cech. 2012; 79(6):506-511 | DOI: 10.55095/achot2012/073

PURPOSE OF THE STUDYRevision total hip arthroplasty is a demanding surgical procedure. It involves a massive loss of acetabular bone stock associated with primary implant removal, and a complicated revision cup fixation in the highly damaged surrounding bone. The authors describe the use of a novel oblong acetabular cup, type TC (Trč-Cingr), for replacement of a loose acetabular component. The aim of the study is to present this novel implant with evaluation of the first clinical results and to report on the authors' experience with revision arthroplasty assessed at 24 to 62 months of follow-up.MATERIALThe TC cup was used in revision total...

Acta Chir Orthop Traumatol Cech. 2012; 79(6):473-483 | DOI: 10.55095/achot2012/069

Ankle fractures are common injuries that require meticulous technique in order to optimise outcome. The Lauge-Hansen and Danis-Weber classifications in addition to careful evaluation of the injury mechanism can help guide treatment but surgeons must be aware that there are injury patterns that will not always fit the afore mentioned patterns. The principles of atraumatic soft tissue handling, rigid internal fixation and early range of motion exercises are critical for successfully treating these injuries. There are still areas of treatment uncertainty and future directed research is needed in order to address some of these questions.

Acta Chir Orthop Traumatol Cech. 2012; 79(6):484-492 | DOI: 10.55095/achot2012/070

The authors discuss arguments concerning indications and selection of implants and operative techniques for arthroplasty in the treatment of femoral neck fractures. Their analysis is based on long-term experience with surgical treatment of patients with hip fractures and on the evaluation of a large number of publications by well-known specialists. The assessed group included 4795 patients treated at their institution between 1997 and 2010, of whom 1532 underwent hip replacement, with 1032 receiving hemiarthroplasty (HA) and 500 having total hip replacement (THR) indicated for femoral neck fractures.A painful hemiarthroplasty due to acetabular...

Acta Chir Orthop Traumatol Cech. 2012; 79(6):493-498 | DOI: 10.55095/achot2012/071

Pediatric pelvic injury is of major significance despite these injuries in children are rare with a suspected yearly rate of 3% of all pelvic injuries. The special pediatric bone anatomy of the pelvis is responsible for different fracture patterns, and overall, a bony or joint injury of the pelvis is an indicator of a severe trauma. The vast majority of pediatric pelvic fractures is the result of a high-energy trauma, especially after strucking by a car or injured as motor vehicle passengers. Additional injuries are common, but additional head injury is only present in 1/3 of patients. An adequate structured primary diagnosis must therefore be...

G. BODE, P. C. STROHM, N. P. SÜDKAMP, T. O. HAMMER

Acta Chir Orthop Traumatol Cech. 2012; 79(6):499-505 | DOI: 10.55095/achot2012/072

Treatment of tibial shaft fractures is still discussed controversial. In the present study current literature was reviewed with the objective to demonstrate current recommendations concerning tibial shaft fractures.Tibial shaft fractures are often caused by high-energy trauma with severe concomitant soft-tissue injuries. Solid bone union without hypertrophy, fast mobilization and full range of motion without further soft-tissue damages are the aims of the therapy. Non-displaced tibial shaft fractures in patients with good compliance can be treated conservatively. Deep venous thrombosis, compartment syndrome, soft tissue injury and chronic regional...

Bilateral L3 Spondylolysis. A Case Report and an Overview of Spondylolytic Defect Repair TechniquesCase report

J. ©RÁMEK, R. BERTAGNOLI, Z. ČEJKA

Acta Chir Orthop Traumatol Cech. 2012; 79(6):549-551 | DOI: 10.55095/achot2012/080

The direct fusion methods for repair of spondylolytic defects of the lumbar spine have recently been replaced by transpedicular screw fixation of the affected segment, in combination with PLIF, TLIF or ALIF procedures. However, in clearly indicated cases, such as a younger patient with no intervertebral disc degeneration and only minimal or no displacement of the vertebra, the direct repair techniques have a great advantage over transpedicular fixation because they preserve segmental motion. The paper reports on a patient with spondylolysis at L3 who underwent surgery combining the Tokuhashi and Matsuzaki and the Gillet and Petit techniques, which...

Are Malleolar Fractures Easy to Treat?In-service-training

P. WENDSCHE, P. DRÁČ

Acta Chir Orthop Traumatol Cech. 2012; 79(6):540-548 | DOI: 10.55095/achot2012/079

Malleolar fractures rank among the most frequent skeletal injuries. The majority of orthopaedic and trauma surgeons incline to surgical therapy with anatomical reposition and retention going for absolute stability. Recommended surgical techniques with screws, plates or tension-band wiring usually presents no problems for surgeons. Unsatisfactory postoperative radiological results, however, are often tolerated because revision surgery may not always have expected results.Difficulties in decision-making for right treatment are (a) the classification and right interpretation of the fracture type, (b) the question of stability and right treatment of...
