Acta Chir Orthop Traumatol Cech. 2018; 85(6):385-391 | DOI: 10.55095/achot2018/066
PURPOSE OF THE STUDYMechanical stimulation plays an uncontested role in the surgical treatment of bone fractures. The effect of frequency, shape, amplitude and rise time of usually symmetrical mechanical stimuli is well known. The recovery period immediately after single pulses is potentially a critical period but has attracted little attention so far. The present study addresses the effect of extending the recovery period which may conceivably increase the efficacy of postoperative fracture management and physiotherapeutic intervention.MATERIAL AND METHODSThe research method consisted of moving an isolated fragment of bone at a fixed amplitude...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):398-404 | DOI: 10.55095/achot2018/068
PURPOSE OF THE STUDYDevelopmental dysplasia of the hip still remains one of the most discussed topics in paediatric orthopaedics. The comprehensive care of paediatric patients with congenital developmental dysplasia of the hip at our department in the period from 1970 to 1985 included, among other things, the open reduction using Ludloff s approach in hip joints where closed reduction was not possible. This technique was supported by some of our own previous observations, such as the original classification of intraoperative findings on acetabular labrum (limbus). This study aims to evaluate the long-term results of treatment of our patients where...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):405-409 | DOI: 10.55095/achot2018/069
PURPOSE OF THE STUDYHip arthroscopy is a widely used method of treatment of hip pathology. It is a technically demanding procedure for which accurate indication is critical. The indication criteria, however, are not yet fully supported by long term results of large patient groups in recent literature. The purpose of our study is to present the 2-year outcomes after hip arthroscopy.MATERIAL AND METHODSIn our study 203 patients (110 women, 93 men) with the mean age of 34 years (17-67 years) were evaluated. The mean NAHS score was 87/100 at 24 months after the surgery, which represents a statistically significant improvement compared to the...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):410-417 | DOI: 10.55095/achot2018/070
PURPOSE OF THE STUDYInformation that would help physicians make decisions and improve the likelihood of achieving the desired results of medical interventions is sought as part of the concept of the individualized approach to patients. The primary purpose of our study was to identify which features determine the higher/lower likelihood of the need for early reoperation after a TKA (total knee arthroplasty). The successful preoperative identification of high risk patients could lead to the adjustment of the surgical procedure and thus lower the percentage of revision surgeries.MATERIAL AND METHODSIn total, 826 patients (296 men and 530 women)...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):418-426 | DOI: 10.55095/achot2018/071
PURPOSE OF THE STUDYNo study has so far been published in the Czech Republic that would try to at least estimate the costs associated with revision total knee arthroplasties (TKA). The purpose of our study was to determine the total costs of primary and revision TKA for aseptic and septic failure and to determine the structure of costs, all with respect to a single workplace in the Czech Republic.MATERIAL AND METHODSThe group included a total of 43 patients aged 55 to 82 years, of whom 23 (53.5%) with primary TKA implantation, 10 (23.3%) patients with revision surgery for aseptic failure, and 10 patients for septic failure of TKA. The costs...
Surgical Treatment of Calcaneus Fractures with C-NailOriginal papers
P. BAŇAŘ, F. BURGET, J. KRAUS
Acta Chir Orthop Traumatol Cech. 2018; 85(6):427-431 | DOI: 10.55095/achot2018/072
PURPOSE OF THE STUDYThe calcaneus bone is the largest tarsal bone of a complex shape, the restoration of which after fracturing, often caused by a high-energy injury, is critical. The top priority in treating these fractures is to correctly asses the condition of the surrounding soft tissues that may be further excessively traumatized by inappropriate timing, surgical approach or technique. Even when adhering to all the rules and guidelines, complications in surgical wound healing have been described in up to 16-33% cases when the extended lateral approach was used. Therefore, the development of minimally invasive techniques, approaches and implants...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):432-437 | DOI: 10.55095/achot2018/073
PURPOSE OF THE STUDYThe success rate of the anterior cruciate ligament (ACL) reconstruction depends on the fixation of the graft, the surgical technique and, of course, on the experience of the operating surgeon. The authors present the development of the construction of the new instrumentation set designed to manage the ACL lesions using the hamstring (HS) tendons. The study is divided into two parts, of which part one focuses on construction, methods and work with new instruments, while part two presents the outcomes of the surgery.MATERIAL AND METHODSFixation of hamstring grafts depends, contrary to the union of bone blocks in the Bone-Tendon-Bone...

Acta Chir Orthop Traumatol Cech. 2018; 85(6):438-440 | DOI: 10.55095/achot2018/074
It is always a big challenge to identify unknown dead persons. All the more so if their body is found decomposed beyond recognition by effects of the environment and decay. Usually, layman and forensic methods (e.g. fingerprint identification) and, in case of the absence of evidence, also medical methods are used for post mortem identification. A method very rarely used in practice is the identification of the deceased through the implants. The described case concerned a finding of a decomposed body of an unidentified man. During the autopsy, apart from the signs of multiple surgeries, a dynamic hip screw implant with a well identifiable serial number...
Spontaneous Consolidation of Exercise-Induced Bilateral Apophyseolysis of Lesser Trochanter in a Young Ice Hockey Player - Long-Term Follow-up by MRICase report
B. PAUČEK, D. SMÉKAL, R. HORÁK
Acta Chir Orthop Traumatol Cech. 2018; 85(6):441-446 | DOI: 10.55095/achot2018/075
Exercise-induced bilateral apophyseolysis of lesser trochanter with enthesopathy of iliopsoas muscle is a less frequent cause of the groin pain syndrome in children and adolescents. In a 13-year-old boy, an active ice hockey player, spontaneous consolidation of exercise-induced bilateral apophyseolysis of lesser trochanter and regression of enthesopathy of iliopsoas muscle is documented by a long-term follow-up through MRI. The conservative treatment comprises targeted rehabilitation focusing on the hypertonic muscles, but of equal importance is also comprehensive rehabilitation focusing on correction of the posture and coordination of the muscle groups....
