Acta Chir Orthop Traumatol Cech. 2017; 84(1):24-29 | DOI: 10.55095/achot2017/003

PURPOSE OF THE STUDYDiabetics may have an increased fracture risk, depending on disease duration, quality of metabolic adjustment and extent of comorbidities, and on an increased tendency to fall. The aim of this retrospective one-centre study consisted in detecting differences in fracture healing between patients with and without diabetes mellitus. Data of patients with the most common fracture among older patients were analyzed.MATERIAL AND METHODSClassification of distal radius fractures was established according to the AO classification. Inital assessment and follow-up were made by conventional x-rays with radiological default settings....

PURPOSE OF THE STUDYPatellar surface replacement during total knee arthroplasty is still a matter of discussion among orthopedic surgeons. The purpose of this study was to examine possible benefits of patellar surface replacement in selected patients with symptomatic degenerative valgus deformity. We have not found any studies in the literature that compare the results of patella management solely for valgus or varus knee deformity nor those that compare both.MATERIAL AND METHODSPatients were randomly assigned to a group that would receive patellar surface replacement during total knee arthroplasty and a group of patients in whom...

Acta Chir Orthop Traumatol Cech. 2017; 84(1):35-39 | DOI: 10.55095/achot2017/005

PURPOSE OF THE STUDYIn the management of dens axis fractures in patients older than 65 years of age the posterior approach is preferred due to osteoporosis and the risk of a failure of anterior osteosynthesis. The posterior approach, however, is associated with a higher incidence of complications.A combination of anterior transarticular fixation of C1/2 (ATS) with compression osteosynthesis of dens axis significantly increases the stability of osteosynthesis.MATERIAL AND METHODSIn the period from 2009 to 2015 our hospital admitted 13 patients older than 65 years of age with a diagnosed type III dens axis fracture based on AO classification....

Acta Chir Orthop Traumatol Cech. 2017; 84(1):40-45 | DOI: 10.55095/achot2017/006

PURPOSE OF THE STUDYTo describe epidemiological and clinical features of pyogenic spinal infections in patients treated at the Department of Infectious, Parasitic and Tropical Diseases of the Hospital Na Bulovce in 2010-2014, and to analyse a predictive significance of selected variables.MATERIAL AND METHODSA single-centre retrospective cohort study carried out from 1.1.2010 to 31.12.2014 enrolled adult patients with septic spondylitis, discitis and facet joint infections. Recorded parameters included: demographics, chronic comorbidities, time to diagnosis, radiological work-up, anatomical level of spinal infection, source of infection,...

Acta Chir Orthop Traumatol Cech. 2017; 84(1):46-51 | DOI: 10.55095/achot2017/007

PURPOSE OF THE STUDYThe study presents the monocentric retrospective study of a group of patients with malignant tumours around the knee, treated by a wide resection and a reconstruction with megaprosthesis due to infectious complications. Provided is a detailed analysis of each operative treatment due to the manifestation and process of periprostethic infection of the knee megaprosthesis and the use of external fixator during a two-stage revision.MATERIAL AND METHODSBetween 01/1993 and 12/2013, a total of 67 cemented megaprostheses were assessed, with a detailed analysis of 12 patients with periprosthetic infection. The Kaplan-Meier...

Acta Chir Orthop Traumatol Cech. 2017; 84(1):52-58 | DOI: 10.55095/achot2017/008

PURPOSE OF THE STUDYThe study consists of a retroactive evaluation of results of surgical treatment in patients with periprosthetic femoral fracture after total hip replacement and a comparison with results reported in the literature.MATERIAL AND METHODSIn the period from 2003 to 2013, a total of 83 patients with periprosthetic femoral fracture after total hip replacement were treated at our clinic, namely 69 women and 14 men. The mean age in the cohort was 74 years (range 47-87). The Vancouver classification was used to grade the fractures. The cohort included 31 patients with type B1 fracture, 25 patients with type B2 fracture, 8 patients...

J. URBAN, P. TOUFAR, M. KLOUB

Acta Chir Orthop Traumatol Cech. 2017; 84(1):59-65 | DOI: 10.55095/achot2017/009

PURPOSE OF THE STUDYThe paper aimed to evaluate the long-term outcomes of the treatment of diaphyseal femur fractures in children treated with Bryant's vertical traction. Moreover, we also assessed the size of overgrowth in the injured femur.MATERIAL AND METHODSThe study included 23 patients with 23 femoral shaft fractures treated with Bryant's vertical traction at our department in 2009-2014. The following parameters were assessed: sex, injured side, weight, age, mechanism of injury, potential abuse, type of fracture, duration of traction, and total length of hospital stay. The healing time of the fracture was the same as the duration...

Acta Chir Orthop Traumatol Cech. 2017; 84(1):13-23 | DOI: 10.55095/achot2017/002

About 90 % of all cases of painful posttraumatic ankle arthritis can be very successfully treated with a minor invasive ankle arthrodesis technique by using a small anterior approach and a fixation with four 6. 5 mm screws of which the posteromedial and transfibular one are inserted percutaneously. The results with this standardized procedure have been reported previously as excellent and good in a mid-term run of 6 years (34). This technique leads to a high union rate of 99% (92 of 93) with rapid bone healing within 8 ± 2 weeks, it causes a low minor complication rate of 8 % and enables a significant increase of the AOFAS ankle/hindfoot score...

L. PA©A, R. VESELÝ, M. KELBL

Acta Chir Orthop Traumatol Cech. 2017; 84(1):66-69 | DOI: 10.55095/achot2017/010

The authors present a rare case of bilateral asymmetric traumatic dislocation of hip joints, where the left joint was treated conservatively after the reduction, while the right joint, with an acetabular fragment interposition, was treated surgically - by arthroscopically assisted reduction and fixation of an osteochondral fragment of posterior wall of the acetabulum. The female patient healed with no complications, showing an excellent clinical outcome with no signs of instability or limited mobility of hip joints, and also with no signs of para-articular calcification or necrosis of the hip at 1 year after the injury and treatment.Bilateral...

Acta Chir Orthop Traumatol Cech. 2017; 84(1):70-73 | DOI: 10.55095/achot2017/011

Our paper presents a unique case of a 64-year-old patient after a fall, treated with oral antidiabetic drugs for type II diabetes mellitus.Following a series of examinations, a bilateral injury was diagnosed - patellar ligament tear on the right side and rupture of quadriceps femoris muscle on the left side. It is a rare injury, complicated by simultaneous involvement of both knee joints. The used therapy consisted of a bilateral surgery followed by gradual verticalisation, first with the support of a walking frame and later with the use of forearm crutches. During the final examination, the patient demonstrated full flexion at both knees, while...
