PURPOSE OF THE STUDYIn certain patients after treated pelvic fractures, heterotopic ossifications can be observed in the area of the pubic bone, which protrude ventrally, and often also laterally and distally into surrounding soft tissues of the groin or even medially into the proximal thigh. These ossifications are shaped like sharp spikes of various lengths, which is why the authors refer to them as "spicules". In some patients, these ossifications are also associated with pain. The aim of this study was to provide an overview of the morphology of heterotopic ossifications of the anterior segment of the pelvis and to discuss the etiology of their...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):260-265 | DOI: 10.55095/achot2021/040
PURPOSE OF THE STUDYTreatment of patella fractures has always been difficult; especially in older patients. The aim of this study was to investigate the XS nail in the treatment of patella fractures.MATERIAL AND METHODS49 fractures could be registered and reevaluated with a standardized questionnaire.RESULTSMean follow-up time was 38.67 months, mean age 71.48 years. The mean OKS was 16.26 points, the Kujala Score was 81.56 points. No wound healing or revision could be detected.CONCLUSIONSThe XS nail is a good treatment option with a low risk of complications also for the age-related trauma patient and for more complex patella...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):266-272 | DOI: 10.55095/achot2021/041
PURPOSE OF THE STUDYThe aim of this study is to show new possibilities for the treatment of anterior cruciate ligament injuries. For many years, the long-established rules have been followed. However, throughout history, treatment strategies have been gradually modified. The advent of new modern technologies now makes it possible to perform reconstructive procedures already in the acute phase, and thus shorten the treatment time significantly.MATERIAL AND METHODS85 patients were followed after an acute knee injury with an isolated anterior cruciate ligament (ACL) lesion. There were 51 men and 34 women with the average age of 32 years. All...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):273-283 | DOI: 10.55095/achot2021/042
PURPOSE OF THE STUDYWe present the results of the retrospective study comparing the clinical outcomes of outside-in versus all-inside suture techniques of unstable RAMP lesions (RL) of the medial meniscus in patients with simultaneous ACL lesion with a minimum two-year follow-up.MATERIAL AND METHODSThe retrospective evaluation covered two groups of patients who underwent the arthroscopic repair of unstable RL in combination with anterior cruciate ligament reconstruction (ACLR). Group 1 included 42 patients (28 men/24 women). Group 2 consisted of 36 patients (21 men/15 women). In Group 1, RL suture repair using the outside-in technique by...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):284-290 | DOI: 10.55095/achot2021/043
PURPOSE OF THE STUDYTo evaluate the mid-term clinical results of patients with Haglund's deformity treated with the central tendon-splitting procedure, and to determine the factors related to patient satisfaction. A retrospective evaluation was made of 20 patients treated with the central tendon-splitting procedure for Haglund's deformity by a single surgeon over a 5-year period.MATERIAL AND METHODSThe patients were evaluated preoperatively and in the postoperative follow-up examinations with the American Orthopedic Foot and Ankle Society (AOFAS) scores and visual analog pain scale scores (VAS). Satisfaction with the surgical results was...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):291-301 | DOI: 10.55095/achot2021/044
PURPOSE OF THE STUDYThe purpose of the study is to verify the correct alignment of components of the Oxford medial unicompartmental knee arthroplasty using the Zimmer Microplasty® instrumentation at the beginning of the learning curve. The implantation of prosthetic components of partial knee arthroplasty in proper alignment has an effect on long-term survival of the prosthesis and should eliminate the occurrence of frequent complications.MATERIAL AND METHODSThe study group includes 20 patients, 9 men with the mean age of 68 years (range 62-78 years) and 11 women with the mean age of 69 years (range 52-81 years). 13 patients underwent...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):302-306 | DOI: 10.55095/achot2021/045
PURPOSE OF THE STUDYThe aim of this study was to evaluate and compare the functional outcomes and complication rates of patients in the mid-term follow-up period when patients applied with medial unicondylar knee arthroplasty (UKA) were grouped according to Body Mass Index (BMI) values.MATERIAL AND METHODSThe study included 125 patients applied with medial UKA between December 2013 and December 2017 because of isolated medial compartment gonarthrosis and completed at least 2 years of follow-up. The patients were separated into three groups: non-obese with a BMI < 30 kg/m2, obese with BMI of 30-39 kg/m2, and morbidly...

INTRODUCTIONThe authors present an overview of polytrauma patients with urinary tract injuries managed in the Level I trauma centre. The purpose of this study was to evaluate the frequency of injuries to the urinary system, the mechanism of their injury and the method of treatment in the cohort of polytrauma patients treated in the trauma centre.MATERIAL AND METHODSBetween 2012 and 2016, a total of 231 polytrauma patients were treated, of whom 47 (20.3%) also suffered a urotrauma. The renal trauma, ureteral trauma, bladder trauma and urethral trauma were classified according to the American Association for the Surgery of Trauma (AAST) scoring...

Acta Chir Orthop Traumatol Cech. 2021; 88(4):313-320 | DOI: 10.55095/achot2021/047
PURPOSE OF THE STUDYIn this study, our aim is to examine the effect of proximal fibular osteotomy on knee and ankle kinematics with finite element analysis method.MATERIAL AND METHODSOne 62-year-old, female volunteer's radiologic images were used for creating lower limb model. Osteotomized model (OM) which was created according to definition of PFO and non-osteotomized model (NOM) were created. To obtain a stress distribution comparison between the two models, 350 N of axial force was applied to the femoral heads of the models.RESULTSAfter PFO, the average contact pressure decreased 26.1% at the medial tibial cartilage and increased...

Large unresectable STS presents a therapeutic challenge. Several options are being explored to avoid amputation without compromising the oncological outcome. Neoadjuvant chemotherapy delivers inconsistent and rather unsatisfactory results, preoperative radiotherapy compromises healing, hence it can impede adjuvant systemic treatment. We present a case report of neoadjuvant use of isolated limb perfusion with TNF-alfa and Alkeran (Melphalan) in a patient with initially unresectable large myxoid liposarcoma of the thigh. We achieved 55% reduction in size of the tumor that allowed for wide resection with a safe margin. Pathology confirmed 99% tumor necrosis....
