I Marík, M Kuklík, J Kraus, A Schejbalová, R Kubát
Acta Chir Orthop Traumatol Cech. 1992; 59(2):67-76
It is a follow-up to Part I of the communication where the authors report on the present possibilities of the diagnostics and therapy of arthrogryposis multiplex congenita. In evaluating the patients with congenital contractures they proceed from the classification of Hall et al. of 1982 and 1983. A 24-member group is made up of patients with classic and distal arthrogryposis. The diagnostics is based on the clinical-genetical examination including dermatoglyphical and electromyographical examination. Electromyographical examination aims at the determination of the prognosis and impairment of mobility. EMG examination has proved neurogenic lesion in...
[Hybrid replacement of the hip joint of the poldi-cech system.].
O Cech, S Beznoska, M Krbec
Acta Chir Orthop Traumatol Cech. 1992; 59(2):77-80
The authors present cementless endoprothesis of the hip joint from the new series of hybrid endoprothesis of the hip joint being developed. The endoprothesis has a conic shaft allowing - with a precise reaming of the medullary canal by means of pneumatic reamer - good primary stability which provides the prerequisites for the remodellation of the bone bed. Endoprothesis is produced from the titanium alloy Ti-5 AI-2,5 Fe. The replacement is indicated for biologically young patients with a solid skeleton. Key words: hybrid endoprothesis, titanium alloy Ti-5 AI-2,5 Fe, cementless endoprothesis.
[Covering skin defects in the area of achilles tendon.].
J Veselý, L Barinka, Z Ziegelbauer, D Ostrizek
Acta Chir Orthop Traumatol Cech. 1992; 59(2):81-85
The authors refer to the possibilities of the treatment of skin defects in the area of Achilles tendon in case of complication after an injury or operation. They discuss advantages and disadvantages, achievements and techniques of treatment with the application of Thiersch skin grafts, cross leg flap, tubulized Filatov flap and free microsurgical transfer. The authors emphasize the advantages of free flap transfer and present three most frequently used types of flaps in this location: forearm flap, the flap of the dorsum of foot and scapular flap. Out of 17 flaps transferred to the lower extremity 6 were used in the area of Achilles tendon with 100%...
[Application of Bone Grafts in the Materials of the Clinic of Children's Orthopaedics in Brno in the Recent 10 Years.].
B Crha, M Janovec
Acta Chir Orthop Traumatol Cech. 1992; 59(2):86-89
Out of the total number of operations performed at the Clinic of Children's Orthopaedics in Brno bone grafts were used in 51.9 cases (9.37%). The authors have divided bone grafts according to their character into autografts, frozen aloimplants and AAA bone, and they also used their combinations. The authors deal with various types of grafts and their role in certain operations. In principle they differentiate 4 basic groups of the clinical application of bone grafts aimed at: 1.fixing the placement of fragments in osteoto mies (276 times) 2.filling in the defects after excochleation of foci of benign tumours and tumour-like lesions (152 times) 3.biological...
[Diagnostics and Express Orthesis in Children's Flat Foot.].
I A Mendelevic, M P Pitkin, V I Ogorodnikov, E E Arzanikovová
Acta Chir Orthop Traumatol Cech. 1992; 59(2):90-92
The method for diagnostics of flat foot in children at a specialized department for orthopaedic two-degree examination is described. The first degree consists in the very finding of flat foot, the second degree in the determination of the construction of the orthopaedic product with the subsequent express orthesis based on a module principle of devising orthopaedic insole. Key words: flat foot in children, modular orthesis.
[Impact of a plaster on the pulmonary functions in idiopathic scolioses.].
I Müller, Z Maska, I Suskevic, J Motycka, M Nýdrle
Acta Chir Orthop Traumatol Cech. 1992; 59(2):93-95
The authors followed up the impact of a plaster applied in idiopathic scolioses on the pulmonary functions measured by microcomputer spirometer. Pulmonary functions were examined in 24 patients prior to the operation of scoliosis and after the operation in the plaster and in 20 patients treated only by the plaster and on 45th day after its removal. The results of both groups were statistically evaluated by a Student t-test at 1 % level of significance by a personal computer Commodore 16. The results of the examination have shown that the plaster as a therapeutical device in idiopathic scolioses considerably restricts the respiratory functions. Simultaneously...

[The present survey of the views on electrostimulation in the treatment of idiopathic scolioses.].
I Müller
Acta Chir Orthop Traumatol Cech. 1992; 59(2):99-101
Proceeding from the existing experience with longterm follow-up of large groups of patients it can be stated that surface electrostimulation of idiopathic scolioses is ineffective. Of some effectiveness is only the ESI method (i.e. an electrode implanted in the muscles). In this it should be mentioned that selected for this treatment were flexible curves with the curvature up to 20 degrees after Cobb at prepubertal age. More severe curves, upper thoracic curves and rigid curves are all contraindicated for the treatment by electrostimulation. The presented survey of the latest works points out also the advantage of early detection, longterm regular...
[Regeneration of bone after osteomyelitis of femur.].
P Vavron, M Doskocil, R Pavlanský
Acta Chir Orthop Traumatol Cech. 1992; 59(2):102-105
The authors describe in their case report the case of regeneration of proximal femur in the patient with peracute hematogenous osteomyelitis. In a 13-year old patient proximal 20 cm of bone were gradually resected, while large sequesters were extirpated with the periost left untouched. After the healing of the acute infect there occurred a quick and quality regeneration of the resected bone. The authors discuss the question of factors governing the regeneration of bone in the typical shape and the relation of the function and structure of the bone in ontogenesis. Key words: acute osteomyelitis, bone regeneration.
[Impairment of the saphenous nerve by a tourniquet.].
P Levkus, J Fagula, M Santa, M Kohút
Acta Chir Orthop Traumatol Cech. 1992; 59(2):106-108
The authors describe a case report where the application of a tourniquet in the area of the Hunter's canal resulted in the impairment of the saphenous nerve with typical symptoms which could be treated only by neurotomy. Examinaton by available clinical methods has not proved other potential causes of the impairment of the nerve. The authors emphasize the significance of acroparesthesiae as the primary symptom of the impairment of the sensitive nerve, they point out the factors of tourniquet tollerance and stress the importance of a proper placement of a tourniquet. Key words: saphenous nerve, tourniquet.
[Horizontal intraarticular dislocation of patella.].
M Kitka, J Franclík, T Molcanyi
Acta Chir Orthop Traumatol Cech. 1992; 59(2):109-111
Horizontal intraarticular dislocation of patella is a rare and relatively unknown injury. The authors describe a case of one patient with this diagnosis. They point out that this type of dislocation of patella should be treated primarily by open reduction with subsequent immobilization by plaster of Paris. The mentioned patient has healed without permanent sequelae. Key words: patella, horizontal dislocation.
[Reconstruction operations versus amputation in the treatment of complex cases of musculoskeletal infection, two case reports with discussion.].
K Foldyna
Acta Chir Orthop Traumatol Cech. 1992; 59(2):112-117
The author presents case reports of two patients with musculoskeletal infection treated in the beginning by complex reconstruction operations which have finally ended up with the amputation of the extremity with a good functional and subjective result. The author deals in details with the problems of differentiated indication of various methods of treatment depending on the condition of the bone, soft tissues as well as the general condition of the patient mainly with regard to the both general and local immunity response and on technical and personnel possibilities of the clinic or hospital. Key words: musculoskeletal infection, reconstruction operations,...
