The article brings the historical survey of evolution of diagnostic procedures and surgical treatment of protruded intervertebral disks. The author comes out partly from the retrieval of English and American literature, which he had chance to obtain from the University library during his study stay in Zürich, partly from the study of Czech and Slovak sources. Historical contribution of the outstanding Czech surgeon, prof. Jan Bedrna, M. D., from Hradec Králové is emphasized. Key words: history, surgical treatment, protruded intervertebral disks.

The objective of this retrospective study is to compare the results of early and delayed surgery of the carpal tunnel syndrome following fracture of the distal radius. The authors evaluate the quality of their treatment in neurological deficit associated with this injury and recommend the correct procedure. In the submitted study 32 patients with carpal tunnel syndrome after fracture of the distal radius operated in 1976-1978 are evaluated. [On the "subvastus" approach in surgery of the knee joint.].

The "subvastus" approach to the knee joint can be considered an alternative to the standard parapatellar approach. It respects anatomical structures to a greater extent, as the insertion of the distal part of the m. vastus medialis is not severed, it preserves better the vascular supply to the patella and makes it possible to test more accurately the movement of the patella in implanted endoprostheses. [Our approach to metacarpophalangeal joints of the long fingers in patients with rheumatoid arthritis.].

In the submitted paper the author describes the surgical indication scheme in deformities of the metacarpophalangeal joint of the long fingers caused by rheumatoid arthritis, as used at the Orthopeadic Clinic, Faculty Hospital, Hradec Králové. The author evaluated a group of patients treated in 1984-1996 where a total of 43 joints were operated. The mean time interval after surgery was 3.5 years. As compared with the world literature, comparable results were achieved, the only complication was a dislocation of the endoprosthesis Rubena II in one patient during the postoperative period. [X-ray Analysis of the Axial Position of the Lower Extremity before and after High Tibial Arcuate Osteotomy.].

The authors investigated by X-ray examinations a group of 36 patients subjected to high tibial arcuate osteotomy. They focus their attention on changes of different angles which in sum form the femorotibial angle. These angles are assessed before surgery and twice after surgery. They found that after six to eight months after surgery when the extremity is fully burdened a partial loss of the correction occurs in the line of osteotomy and this is sometimes associated with a poor subjective result as conceived by the patient. [Early treatment of unstable chest in patients with multiple injuries.].

The authors analyze a group of 11 patients with unstable chest injuries who were subjected in 1994-1996 to internal surgical stabilization of the chest wall. They compare them with a group of 11 consecutive patients from the previous period who were treated by artificial pulmonary ventilation. The basic indicator for evaluation of both groups was the necessary length of assisted ventilation which had an impact on all important parameters, complications and mortality of patients with this injury. [Chronic recurrent multifocal osteomyelitis.].

The authors discuss the diagnostic and prognostic problem of chronic recurrent osteomyelitis (CRMO) described for the first time in 1972 by Giedione. This relatively rare disease of unknown origin is characterized by a slow onset with oedema and pain at several sites of the locomotor apparatus which occur concurrently or consecutively and are followed by relapses. The body temperature is normal or slightly elevated. CRMO is sometimes associated also with palmoplantar pustulosis. The authors submit an analysis of the nutritional status assessed by anthropometric methods in a clinical group of 70 patients (mean age 58 years) where on account of the X-ray and the clinical finding of arthritis of the knees an alloplastic operation was performed. The results of the examination are compared with the analysis of the nutritional status of a group of 20 without X-ray and clinical signs of arthritis of the knee. Based on an analysis of a group of 183 patients, subjected to arthrographic examination and subsequent surgical revision the authors try to draw attention to the effectiveness of radiodiagnostic contrast techniques when evaluating damage of "soft" structures of the knee joint. For evaluation a three-year period from 1993 to 1995 was selected. The authors investigated the correlation between the arthrographic image and the subsequent finding on surgery. [Arthroscopic deliberation of joint.].

The paper deals with new possibilities how to use the arthroscopic surgical technique to relax stiffness of joints of different etiology. The described surgical technique is an alternative of forceful redresses under general anaesthesia and draws attention to the obvious advantages and minimalization of complications. Early rehabilitation is an integral part of the arthroscopic deliberation operation and is made on a motor splint immediately after surgery.
