[The septic knee - arthroscopic treatment.].
V Pavlovcic
Acta Chir Orthop Traumatol Cech. 1999; 66(4):203-205
Arthroscopic irrigation has proved very effective in the treatment of the septic knee, provided that it is begun at the earliest possible time. We obtained very good results in 25 out of 27 cases treated by radical irrigation in the first three days after the start of infection. Cases where the infection had been present for more than six days were managed by synovectomy. Surgical therapy must be accompanied by intensive CPM treatment, combined with isometric exercises for the quadriceps muscle in order to achieve a good functional outcome. Key words: septic knee, arthroscopic irrigation.
[Topography of prevertebral structures in thoracic and lumbar spine.].
A Doubková, M Krbec, J Stulík, H Stenhová
Acta Chir Orthop Traumatol Cech. 1999; 66(4):206-212
Authors describe the topographic relation of the anterior aspect of thoracolumbar spine to individual prevertebral structures (vessels, nerves, ligaments) as their injury may cause serious complications during spine surgery. Observations have been made on cadavers and checked by peroperative findings and findings on MRI sections. Having a good view of prevertebral anatomical structures has a principal significance for the use of anterior surgical approaches. Key words: topographic anatomy, thoracolumbar spine, MRI.
[Significance of blunt injury of thorax in polytrauma.].
F Vyhnánek, J Fanta, P Lisý, R Kostka, T Vedra, O Vojtísek
Acta Chir Orthop Traumatol Cech. 1999; 66(4):213-216
Based on the analysis of a group of 100 patients with polytrauma evaluation was made of the impact of thoracic injury on post-injury period, including the incidence of complications, and on the outcome of the treatment. Blunt thoracic injury occurred in 64 patients. The most frequent complications of intrathoracic injury were pneumothorax, haemothorax and contusion of lungs. In the group of patients with polytrauma involving also the chest there was a more frequent incidence of respiratory insufficiency, higher morbidity (higher incidence of insufficiencies of organs and septic complications) and higher mortality (34%). The cause of mortality within...
[Ultrasonography of rotator cuff of shoulder joint - examination technique, physiological and pathological findings.].
I Bajdová
Acta Chir Orthop Traumatol Cech. 1999; 66(4):217-224
The author presents basic information on ultrasonographic examination of the shoulder joint. She mentions the beginnings of the development of ultrasonography of the shoulder, puts emphasis on the technique of this examination and describes the procedure of the examination of the rotator cuff of the shoulder joint. In the conclusion she highlights the importance of this method as a dynamic procedure allowing to monitor the development of changes in the region of the rotator cuff and determine in time the appropriate method of treatment. On the group of 27 patients she evaluates 99% sensitivity, 100% specificity and 93% accuracy of this examination....
[A Tennis Elbow, Results of Surgical Treatment by a Modified Boyd McLeod Procedure.].
I Vanecek
Acta Chir Orthop Traumatol Cech. 1999; 66(4):225-230
The author present results of surgeries by the modified Boyd-McLeod procedure performed in the period 1985-1995. He evaluated results of 285 operations and compared them with the results of 54 operations performed by Hohmann procedure in 1982-1984 at the same clinic. He also compares the results with other surgical methods of different authors. However, the comparison is difficult due to different criteria used for the evaluation of the results of operations. Excellent and good results were achieved in 88 % cases by the modified Boyd-McLeod procedure as compared to only 79 % of excellent and good results of Hohmann procedure. In contrast to the original...
[Early mobilization in patients with intertrochanteric fracture.].
J Vojtassák, M Lisý, M Demetrovic
Acta Chir Orthop Traumatol Cech. 1999; 66(4):231-234
In the article the authors present results of surgical treatment of intertrochanteric fractures at their clinic with the focus on an early post-operative mobilization. In the oldest age group which involves the greatest risk as concerns the general health condition they seek such a type within the indication of surgical treatment which would allow an early mobilization. Osteosynthesis makes possible a good reduction, however, it is necessary to avoid full weight bearing, mainly in case of osteoporosis, which elderly patients are not able to manage. Hemiarthroplasty requires to reconstruct the femoral calcar by means of cementing or to apply wire loop...
[Analysis of group of patients treated in 1997 for proximal femur fractures.].
V Dzupa, J Bartonícek, J Skála-Rosenbaum
Acta Chir Orthop Traumatol Cech. 1999; 66(4):235-239
The authors present an overview of basic demographic data obtained from the follow up of 248 patients treated for a proximal femur fracture at their clinic in 1977.The average age (77 years) and the predominance of patients older than 70 years (81 %) are comparable with the groups of patients presented in the foreign literature. The group of patients accounts for approximately 2 % of the national group (data acquired from the Institute of Health Care Information and Statistics), the average age is by 5 years higher and the predominance of patients older than 70 years is more significant by approximately 10 %. Also the percentage of mortality in the...
[Treatment of the rupture of ulnar collateral ligament of metacarpophalangeal ligament of the thumb, surgical treatment of chronic instability.].
M Lutonský
Acta Chir Orthop Traumatol Cech. 1999; 66(4):240-242
The author presents possible solutions of both acute rupture and chronic lesion of metacarpophalangeal ligament of the thumb of the hand. Chronic post-trauma instability is indicated by the author for modified reconstruction of the ligament after Strandell. Since 1991 such operation was performed in 12 patients who were followed with an average interval 3 years after the surgery. The mentioned evaluation scheme showed 8 excellent and 4 good results of the operation. Key words: metacarpophalangeal joint, ulnar collateral ligament, reconstruction of ligament rupture.
[Percutaneous transfixation in fractures of distal radius and evaluation of results.].
L Tomcovcík, J Kubasovský
Acta Chir Orthop Traumatol Cech. 1999; 66(4):243-247
Authors evaluate results of fractures of distal radius in a group of patients who were treated with the method of percutaneous osteosynthesis by means of K-wires.Two systems were used in the evaluation. Using the Green and O'Brien evaluation system modified by Cooney excellent and good results were achieved in 54,2 % patients. On the basis of Gartland and Werley evaluation system specified by Sarmiento excellent and good results were achieved in 75 % patients. Authors point out the difference in the results caused by a greater strictness of the first evaluation system mentioned. Key words: fractures of distal radius, transfixation.
[Osteoid Osteoma and Lumbar Non-structural Scoliosis.].
I Müller, O Vlach, J Cienciala
Acta Chir Orthop Traumatol Cech. 1999; 66(4):248-250
A benign bone lesion osteoid is characterized by sclerotic bone interface with a central nidus and pain at night which can be influenced by salicylates. When located on vertebrae or ribs it may cause non-structural scoliosis. Authors describe a case of 11-year old girl suffering from pain in lumbar spine at night, with left-side non-structural scoliosis which was unsuccessfully treated by physical therapy. Subsequently she was handled by radical resection of the L5 pedicle on the right side where osteoid osteoma was revealed by means of CT and later verified by histology. Unfortunately, minor non-structural scoliosis of lumbar spine persist also after...
