PURPOSE OF THE STUDYCervical spondylotic myelopathy is the most common cause of spinal cord dysfunction in patients over 60 years old. Symptoms often develop gradually and insidiously and are characterized by neck stiffness, arm pain, numbness and clumsiness of hands, as well as weakness of the hands and legs frequently leading to a change in mobility. Surgery is performed primarily to prevent the progression of symptoms but also with the aim of improving existing symptoms.Aim of our study was to assess the outcomes and potential complications of surgical decompression of cervical spondylotic myelopathy (CSM).MATERIAL AND METHODSProspective...

PURPOSE OF THE STUDYType B3 thoracic and lumbar fractures are often found in spines with previous hyperossification processes such as ankylosing spondylarthritis (AS) or diffuse idiopathic skeletal hyperostosis (DISH). They occur most frequently due to high-energy trauma in a healthy spine and as fall-related domestic injury in a spine affected by hyperossification. Generally, they are less frequent than type B3 cervical spine fractures. In this retrospective study involving two centres, the incidence of these fractures, their characterisation, therapy and complications associated with them were investigated.MATERIAL AND METHODSBetween...

PURPOSE OF THE STUDYThe aim of this prospective study was to investigate whether female gender and revision surgery were significant risk factors for intra-operative periprosthetic fractures during total hip arthroplasty (THA).MATERIAL AND METHODSThe group investigated comprised the patients who, in the period 1995-2009, sustained an intra-operative periprosthetic fracture during primary or revision THA. The patients were treated by a therapeutic procedure based on the Vancouver system. The results were related to the total number of patients undergoing THA in that period, Statistical analysis was performed using Pearson's χ2...

PURPOSE OF THE STUDYLeukocyte esterase is an enzyme in neutrophils from which it is released into exudate; its detection by colorimetric test strips indicates the presence of neutrophils. This is a rapid method to find whether exudate is of infectious or non-infectious aetiology. The aim of the study was to determine the sensitivity and specificity of leukocyte esterase testing with use of AUTION Sticks (Arkray) for examination of exudates obtained in inflammatory diseases of the skeletal system.MATERIAL AND METHODSExudates associated with skeletal system diseases were collected from 45 patients in the period from July 1st to December 31st,...

PURPOSE OF THE STUDYEvaluation of infectious complications in patients with proximal femoral fractures treated by osteosynthesis using dynamic hip screws (DHS).MATERIALThe group included 501 patients with 532 DHS procedures performed in the years 1996-2010. In 31 patients osteosynthesis was carried out bilaterally.METHODSOsteosynthesis was indicated for femoral neck fracture in 18 hips and for pertrochanteric fracture in 514 hips. Prophylactic antibiotic therapy was administered within 48 hours of surgery. The occurrence of infectious complications (surface and deep wound infection), presence of infectious agents, risk factors,...

The evaluation of present long-term studies on results after surgical stabilization of the pelvic ring is difficult, as different treatment concepts are used and the majority of these studies are not comparable regarding selected evaluation parameters. Additionally, no standardized measurement instrument exists to analyze the clinical and radiological result after pelvic ring injuries.Only short-term evaluations with a mandatorily recommended minimum follow-up time of one year are available. Medium-term analyses or real long-term analyses are missing.Present data show an increase of long-term sequelae from stable type A injuries to completely...

Fractures of the shaft of the humerus are common injuries. Historically, the treatment of choice has been functional bracing. Recent technical advances such as angular stable plate and nail constructs lead to a shift in the treatment algorithm for this type of fracture. Surgical treatment is recommended in high grade open fractures, multilevel injury, polytrauma patients, nerve and serious vascular injuries, pathological fractures, and delayed or nonunions. This article aims to describe the published scientific data and current treatment modalities most suitable for each type of fracture.

Ultrasonography is an excellent tool for diagnosing pathologies of peripheral nerves. It quickly provides reliable information about possible nerve entrapment, even in post-operative conditions. We present a case of failed surgery for carpal tunnel syndrome in which ultrasonography gave us immediately all the information needed for quick therapeutic decision-making. Some basic principles of ultrasonography of peripheral nerves are also mentioned.
