PURPOSE OF THE STUDYThe number of patients who underwent total knee replacement is on an increase globally. According to the current literature, 10-30% of patients is dissatisfied with the outcome of surgery. The purpose of this study was to focus on preoperative planning of femoral components position relative to the original anatomy and to find out how it affects the satisfaction of patients after the surgery.MATERIAL AND METHODSA total of 74 total knee replacements were examined at the authors' department one year after the surgery in order to determine the function of knee replacement. The patients underwent a preoperative and postoperative...

PURPOSE OF THE STUDYThe purpose of the study is to share the first experience of authors with the management of glenoid defects by a bone graft in reverse total shoulder arthroplasty.MATERIAL AND METHODSEvaluation of 7 patients was conducted, who underwent reverse total shoulder arthroplasty at the authors' department. Due to the presence of glenoid bone defect, augmentation was performed by bone graft, obtained either as an autograft harvested from the humeral head of the patient intraoperatively or as an allograft from the femoral head from the bone tissue bank. In all the implantations, a glenoid component with an extra-long "revision"...

INTRODUCTIONVitamin D-deficiency is known to cause nerve conduction impairments, cancer and chronic diseases, as well as the pathogenesis of osteoarthritis. Our goal with this study is to evaluate the cartilage healing by applying intraarticular 1α, 25 (OH) 2D3 at different doses in rats with normal vitamin D levels and metabolism, which we made focal chondral damage model in the knee joint.MATERIAL AND METHODS35 male Sprague-Dawley rats aged 20-24 weeks were used in our study. Both knees of rats were cartilage defected surgically on day 0. Joint injections performed at 06:00 am on 0th and 2nd days and after second injection others...

PURPOSE OF THE STUDYMinimally invasive posterior lumbar interbody fusion and percutaneous instrumentation were introduced in the clinical practice with the aim to reduce the damage to musculoligamentous structures associated with open surgeries. The purpose of the study is to evaluate the clinical outcomes, radiological results and complications of the group of patients operated with the use of the minimally invasive posterior lumbar interbody fusion technique.MATERIAL AND METHODSThe group of 31 patients operated for lumbar degenerative disc disease was followed-up prospectively. The surgical technique included a unilateral microsurgical...

PURPOSE OF THE STUDYComparison of the outcomes of clavicle midshaft fractures using two different surgical techniques, namely intramedullar osteosynthesis using elastic stable intramedullary nailing (ESIN) with medial cap and extramedullar plate osteosynthesis.MATERIAL AND METHODSThe prospective randomized trial was conducted at the Department of Trauma Surgery of the Department of Surgery of the University Hospital Hradec Králové in the Czech Republic at the Level I Trauma Center between 2014 and 2018 and compared two types of osteosynthesis of clavicle midshaft fractures. 60 patients were enrolled in the study and were randomly assigned...

PURPOSE OF THE STUDYThe clinical prospective study presents the results of minimally invasive harvesting of cancellous bone tissue in trauma indications. We focused on evaluating the clinical outcomes of this technique, particularly on the amount of cancellous bone harvested, the effectiveness of its use and complications. The pain in the bone graft harvest site is evaluated and compared with the pain after a standard harvest from the iliac crest.MATERIAL AND METHODSAll the patients aged 18-90 years, in whom cancellous bone was harvested by minimally invasive technique using a bone cutter were included prospectively in the study. It was...

PURPOSE OF THE STUDYMIPO (Minimally Invasive Plate Osteosynthesis) technique is currently a commonly used method to treat fractures of the distal third of tibia diaphysis. At the same time, it is just like other methods accompanied by a relatively high complication rate. The purpose of this study was to determine whether the use of this technique at our department is effective and whether the complication rate does not differ considerably from values reported by literature.MATERIAL AND METHODSIn the period 2014-2016, minimally invasive plate osteosynthesis was performed in a total of 42 patients, who had suffered a fracture of the distal...

PURPOSE OF THE STUDYThe purpose of the study is to verify the sensitivity of pre-hospital triage algorithm used in the Czech Republic, which decides on directing the patients at risk of a failure of vital functions into a trauma centre. Another aim is to find out whether the triage algorithm extension by items F4-persistent traumatic paralysis and M7-buried under heavy objects, implemented in 2015, resulted in an increased sensitivity of triage.MATERIAL AND METHODSIt is a retrospective, observational, monocentric study. Included in the study were all the trauma patients with the National Advisory Committee on Aeronautics (NACA) score 3-6...

Patellar sleeve fracture is a form of injury in which small osseous fragments avulsed with periosteum and cartilage.15-year-old male patient, playing in school football team, apllied to our clinic with a history of previously missed patellar superior pole sleeve avulsion fracture.Care must be taken in order not to miss the patellar superior pole sleeve fractures, which are very rare in children. Extra care must be taken in patients, whose X-ray imaging is clean but there is a problem in the extensor mechanism of the knee.

Heparin-induced thrombocytopenia is a rare complication of treatment with both unfractionated heparin (UFH) and low molecular weight heparin (LMWH). Antibodies against the complex heparin-platelet factor 4 are the main cause of pathogenesis, resulting in the activation of thrombocytes, coagulation, endothelium, monocytes, neutrophils and subsequent highly prothrombotic state. The prothrombotic state can result not only in venous but also in arterial thrombosis at different locations (which is manifested apart from venous thromboembolic disease also by acute limb ischemia, acute myocardial infarction, ischemic stroke, skin necrotizing lesion exanthema)....
